摘  要:目的探讨加味二陈汤对慢性阻塞性肺疾病大鼠肺功能及平滑肌细胞(ASMCs)作用的相关机制。方法选择50只SPF级雄性大鼠,其中40只建立慢性阻塞性肺疾病大鼠模型,其余10只不做处理,分正常对照组、模型对照组、加味二陈汤5、10、20 g/kg组,苏木素-伊红(HE)染色观察慢性阻塞性肺疾病大鼠肺组织病理变化,记录气道平滑肌厚度及肺功能,RT-聚合酶链反应(PCR)、Western印迹检测ASMCs中Janus激酶(JAK)3、转录激活子(STAT)3 mRNA及蛋白表达。结果与正常对照组比较,模型对照组肺功能显著降低,用力呼气量/用力肺活量(FEV0.3/FVC)、动态肺顺应性(Cdyn)值显著降低,呼吸指数(RI)明显升高(P<0.001);与模型对照组比较,加味二陈汤5、10、20 g/kg组肺功能均明显优于模型对照组,且加味二陈汤20 g/kg组肺功能改善最显著(均P<0.05)。与正常对照组比较,模型对照组气道平滑肌厚度显著提高(P<0.001);与模型对照组比较,加味二陈汤5、10、20 g/kg组气道平滑肌厚度明显降低,且加味二陈汤20 g/kg组气道平滑肌厚度降低最显著(P<0.05),提示慢性阻塞性肺疾病大鼠气道壁显著增厚。正常对照组肺泡结构完整,无毛细血管出血、水肿、炎性细胞出现;模型对照组肺泡结构不完整,大小不均,肺组织间有轻微毛细血管充血,肺间质轻微水肿,肺泡腔也存在炎性细胞;加味二陈汤组肺泡结构、毛细血管充血程度、肺间质水肿程度及炎性细胞浸润均有所改善。与正常对照组比较,模型对照组ASMCs中JAK3、STAT3 mRNA及蛋白表达明显增加(P<0.001);与模型对照组比较,加味二陈汤5、10、20 g/kg组ASMCs中JAK3、STAT3 mRNA及蛋白表达明显降低(P<0.05)。结论加味二陈汤对慢性阻塞性肺疾病大鼠有明显治疗作用,能显著提高大鼠肺功能,其作用机制与抑制ASMCs中JAK/STAT信号活性有关。Objective To investigate the action mechanism of Modified Erchen decoction on pulmonary function and smooth muscle cells(ASMCs)in rats with chronic obstructive pulmonary disease.Methods 50 SPF grade male rats were selected,40 of which were established chronic obstructive pulmonary disease model and the other 10 rats were not treated.40 rats were divided into normal control group,model control group,Modified Erchen decoction 5,10 and 20 g/kg groups.Hematoxylin-eosin(HE)staining was used to observe the pathological changes of lung tissue in rats with chronic obstructive pulmonary disease.The thickness of airway smooth muscle and lung function of rats were recorded.The mRNA and protein expressions of Janus kinase(JAK)3/activator of transcription(STAT)3 in ASMCs were detected by RT-polymerase chain reaction(PCR)and Western blot.Results Compared with the normal control group,the lung function of the model control group was significantly decreased,forced expiratory volume/forced vital capacity(FEV0.3/FVC)and dy-namic lung compliance(Cdyn)values were significantly decreased,and respiratory index(RI)was significantly increased(P<0.001).Compared with the model control group,the lung function of Modified Erchen decoction 5,10 and 20 g/kg groups were significantly better,and the lung function was most significantly improved in Modified Erchen decoction 20 g/kg group(all P<0.05).Compared with the normal control group,the thickness of airway smooth muscle in the model control group was significantly increased(P<0.001).Compared with the model control group,the thickness of airway smooth muscle in 5,10 and 20 g/kg group of Modified Erchen decoction were significantly decreased,and the thickness of airway smooth muscle in 20 g/kg group of Modified Erchen decoction decreased most significantly(P<0.05),indicating that the airway wall of rats with chronic obstructive pulmonary disease was significantly thickened.The alveolar structure of rats in normal control group was complete,without capillary hemorrhage,edema and inflammatory c
