摘 要:目的:研究超声多普勒参数主肺动脉加速时间与射血时间比值对妊娠糖尿病孕妇胎儿肺成熟度评估的价值。方法:选取2020年1月—2021年12月期间佛山市妇幼保健院收治的妊娠糖尿病37周后待产孕妇,共计500例,以分娩后胎儿肺成熟度情况,分为胎儿肺成熟组及胎儿肺非成熟组。对所有产妇均采用彩色多普勒超声诊断仪进行检查,期间记录胎儿主肺动脉中段的加速时间(AT)、射血时间(ET),分析AT/ET值对新生儿肺成熟度的预测价值。结果:胎儿肺非成熟组AT/ET值低于胎儿肺成熟组(P<0.05);ROC曲线显示,胎儿主肺动脉中段AT/ET预测新生儿肺成熟度的最佳截断值为0.218时,灵敏度为97.44%,特异度为99.76%,准确率为99.40%,AUC为0.908。通过一致性检验(Kappa检验)分析,AT/ET值预测妊娠糖尿病患者新生儿肺成熟度的效能较高(Kappa值=0.846,P<0.05)。结论:通过超声多普勒参数AT/ET值对妊娠糖尿病孕妇胎儿肺成熟度评估时,能够为临床提供有价值的信息,对新生儿肺成熟度评估有一定预测价值。Objective To study the value of the ratio of main pulmonary artery acceleration time to ejection time in the evaluation of fetal lung maturity in pregnant women with gestational diabetes mellitus.Methods A total of 500 pregnant women waiting for delivery after 37 weeks of gestational diabetes mellitus admitted to Foshan Maternal and Children Hospital from January 2020 to December 2021 were selected.To determine the fetal lung maturity status after delivery.They were divided into fetal lung maturity group and fetal lung immaturity group.All puerperas were examined by color Doppler ultrasound,during which the acceleration time(AT)and ejection time(ET)of the middle segment of fetal main pulmonary artery were recorded,and the predictive value of AT/ET value in neonatal pulmonary maturity was analyzed.Results The AT/ET data of the fetal lung immaturity group was lower than that of the fetal lung maturity group.The ROC curve showed that the best cut-off value of AT/ET in the middle part of the fetal main pulmonary artery to predict neonatal lung maturity was 0.218,the sensitivity was 97.44%,the specificity was 99.76%,and the accuracy was 99.40%.The AUC was 0.908.Through the analysis of consistency test(Kappa test),AT/ET value was more effective in predicting neonatal lung maturity in patients with gestational diabetes mellitus(Kappa value=0.846,P<0.05).Conclusion The ultrasonic Doppler parameter AT/ET value can provide valuable information for clinical evaluation of fetal lung maturity in pregnant women with gestational diabetes mellitus,and has a certain predictive value for the evaluation of neonatal lung maturity.
