苏州某三级公立医院住院超30天患者回顾性分析

摘　　要：目的对苏州市某三级公立医院住院时间超过30天患者分布特征进行回顾性分析,探究影响患者超长住院的因素,提出可行性改进方案及对策,加强医疗质量管理,保障患者安全。方法通过病案首页信息及科室上报信息选取2018—2021年期间共6071人次住院时间超过30天的患者。结果住院超30天患者中男性多于女性,年龄(62.95±15.30)岁。分析显示转科、临床路径、手术次数对住院天数的影响有统计学意义。结论医院应从重视住院超30天患者过程管理、拓展医联体内下转途径、实时监控非计划再次手术、建立手术时长危急值、规范临床路径管理等方面着手,加强医疗质量管理,缩短住院时间,提升医疗效率。Objective A retrospective analysis was made on the distribution characteristics of patients hospitalized for more than 30 days in a tertiary public hospital in Suzhou to explore the factors that affect the extra-long hospitalization of patients,propose feasible improvement plans and countermeasures,and to strengthen medical quality management to ensure patient safety.Methods 6071 patients with more than 30 days of hospitalization from 2018 to 2021 were selected through the information on the first page of the medical record and the information reported by the department.Results More men than women were hospitalized for more than 30 days,the age was 62.95±15.30.The analysis showed that the factors such as whether to transfer to another department,clinical pathway enrollment,and number of operations had a statistically significant impact on the number of days in hospital.Conclusion Hospitals should attach importance to the process management of patients hospitalized for more than 30 days,expand the downward transfer path within the medical association,real-time monitoring of unplanned reoperation,establish the critical value of surgical duration,and standardize clinical path management to strengthen medical quality management,shorten hospitalization time,and improve medical efficiency.
