摘 要:目的探究围绝经期盆底功能障碍性疾病(PFD)患者阴道脱落细胞成熟度、雌激素(E_(2))和25羟基维生素D[25⁃(OH)⁃D]与阴道壁松弛素受体(LGR7)信使核糖核酸(mRNA)的关系。方法选取2020年9月至2022年9月于廊坊市人民医院就诊的围绝经期PFD患者42例,围绝经期健康女性38名,计算阴道脱落细胞成熟指数,检测血清E_(2)、25⁃(OH)⁃D水平以及阴道壁组织LGR7 mRNA相对表达量,分析LGR7 mRNA与其余指标的相关性。结果观察组绝经率、盆底肌静息肌电值、合并阴道炎率、LGR7 mRNA表达水平高于对照组,盆底肌最大收缩力、持续收缩力、阴道脱落细胞成熟度、E_(2)和25⁃(OH)⁃D水平低于对照组,差异均有统计学意义(P<0.05);阴道脱落细胞成熟度、E_(2)和25⁃(OH)⁃D均与盆底肌力呈正相关(P<0.05),LGR7 mRNA与盆底肌力,阴道脱落细胞成熟度、E_(2)和25⁃(OH)⁃D与LGR7 mRNA呈负相关(P<0.05);阴道脱落细胞成熟度、E_(2)、25⁃(OH)⁃D、LGR7 mRNA的曲线下面积分别为0.918、0.919、0.898、0.858(P<0.05);阴道脱落细胞成熟度<58.95%、E_(2)<65.76 pmol/L、25⁃(OH)⁃D<28.19μg/L、LGR7 mRNA>0.58为围绝经期PFD的危险因素(P<0.05)。结论围绝经期PFD患者阴道脱落细胞成熟度、E_(2)和25⁃(OH)⁃D与LGR7 mRNA呈负相关,上述指标均对PFD的诊断具有一定参考价值。Objective To explore the relationship of maturity of vaginal exfoliated cell,estrogen(E_(2))and 25⁃hydroxyvitamin D[25⁃(OH)⁃D]with vaginal wall relaxin receptor(LGR7)messenger ribonucleic acid(mRNA)in patients with perimenopausal pelvic floor dysfunction(PFD).Methods 42 patients with peri⁃menopausal PFD who came to Langfang People's Hospital for treatment and 38 perimenopausal healthy women were selected between September 2020 and September 2022.The maturity index of vaginal exfoliated cell was calculated,and the levels of serum E_(2) and 25⁃(OH)⁃D and relative expression level of LGR7 mRNA in vaginal wall tissue were detected,and the correlation between LGR7 mRNA and other indicators was analyzed.Results The menopause rate,resting electromyography value of pelvic floor muscle,vaginitis rate and expres⁃sion level of LGR7 mRNA in the observation group were higher than those in the control group while the maxi⁃mum contractile force and continuous contractile force of pelvic floor muscle,maturity of vaginal exfoliated cell and levels of E_(2) and 25⁃(OH)⁃D were lower than those in the control group(P<0.05).The maturity of vagi⁃nal exfoliated cell,E_(2) and 25⁃(OH)⁃D were positively correlated with pelvic floor muscle strength,and the LGR7 mRNA was negatively correlated with pelvic floor muscle strength and the maturity of vaginal exfoliated cell,E_(2) and 25⁃(OH)⁃D were also negatively correlated with LGR7 mRNA(P<0.05).The areas under the curves of vaginal exfoliated cell maturity,E_(2),25⁃(OH)⁃D and LGR7 mRNA were 0.918,0.919,0.898 and 0.858 respectively(P<0.05).The maturity of vaginal exfoliated cell<58.95%,E_(2)<65.76 pmol/L,25⁃(OH)⁃D<28.19μg/L and LGR7 mRNA>0.58 were the risk factors of perimenopausal PFD(P<0.05).Conclusion The vaginal exfoliated cell maturity,E_(2) and 25⁃(OH)⁃D are negatively correlated with LGR7 mRNA in patients with perimenopausal PFD,and the above indicators have certain reference value in the diagnosis of PFD.
