皮肤糖基化终产物与2型糖尿病患者心脏自主神经病变相关性的研究

摘　　要：目的探讨皮肤AGEs与T2DM患者心脏自主神经病变(CAN)的相关性。方法选取2020年11月至2022年7月于合肥市第二人民医院内分泌科住院治疗的T2DM患者172例,根据Ewing试验结果分为糖尿病心脏自主神经病变组(DCAN,n=110)和单纯T2DM组(n=62),分析两组临床特征及DCAN的影响因素。结果与T2DM组比较,DCAN组合并DR比例、FPG、HbA1c及皮肤AGEs升高,2 hC-P、胰岛β细胞功能指数降低(P<0.05)。Logistic回归分析显示,合并DR、FPG、皮肤AGEs是DCAN的影响因素。受试者工作特征(ROC)曲线分析显示,合并DR、FPG、皮肤AGEs的ROC曲线下面积为0.632、0.623、0.709,敏感度为40.91%、79.09%、62.73%,特异度为85.48%、40.32%、72.59%。结论合并DR、FPG、皮肤AGEs是DCAN的影响因素,皮肤AGEs对预测DCAN有一定价值。Objective To investigate the relationship between advanced glycation end products(AGEs)and cardiac autonomic neuropathy(CAN)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 172 patients with T2DM hospitalized in the Department of Endocrinology,Hefei Second People’s Hospital were enrolled in this study from November 2020 to July 2022 and divided into two groups according to the results of Ewing test:diabetic cardiac autonomic neuropathy group(DCAN,n=110)and simple T2DM group(n=62).The clinical characteristics,related factors and risk factors for DCAN were analyzed in the two groups.Results Compared with T2DM group,ratio of DCAN combined with DR,FPG,HbA_1c and skin AGEs increased,while 2 hC-P and HOMA-βdecreased in DCAN group(P<0.05).Logistic regression analysis showed that the combination of DR,FPG and skin AGEs were the influencing factors for DCAN.The analysis of subjects'working characteristics(ROC)curve showed that the areas under ROC curve combined with DR,FPG and skin AGEs were 0.632,0.623 and 0.709,with sensitivities 40.91%,79.09%and 62.73%,and specificities 85.48%,40.32%and 80.32%respectively.Conclusion DR,FPG and skin AGEs were the influencing factors for DCAN,skin AGEs has a certain predictive value in the diagnosis of DCAN.
