拉贝洛尔、依诺肝素联合治疗妊娠期高血压的临床疗效及对妊娠结局的影响

摘　　要：目的探讨拉贝洛尔、依诺肝素联合治疗妊娠期高血压的临床疗效及对妊娠结局的影响。方法选取2018年2月-2021年1月在天津市北辰医院治疗的94例妊娠期高血压患者,随机分为观察组和对照组,每组各47例,两组患者均接受常规干预,包括低盐饮食、卧床休息、镇静、吸氧、血压监测、胎心监护等。同时,对照组患者使用拉贝洛尔片,口服,100mg/次,3次/d。观察组患者在拉贝洛尔片基础上,联用依诺肝素,于脐周2cm外皮下注射,4000IU/次,2次/d,治疗5d,停药2d,循环给药。对比两组患者治疗前后的血压指标、24h尿蛋白量及不良妊娠结局有无差异。结果治疗后,观察组患者的收缩压、舒张压水平以及24h尿蛋白水平均明显低于对照组(P<0.05);与对照组患者对比,观察组患者的早产、胎儿窘迫、产后出血发生率更低(P<0.05)。结论拉贝洛尔+依诺肝素治疗妊娠期高血压,在平稳降低患者血压的同时,还减少了不良妊娠结局风险。Objective To investigate the effect of labetalol and enoxaparin combined therapy on gestational hypertension and pregnancy outcome.Methods From February 2018 to January 2021,94 patients with gestational hypertension treated in our hospital were selected and randomly divided into observation group and control group,47 patients in each group received routine intervention,including lowsalt diet,bed rest,sedation,oxygen inhalation,blood pressure monitoring,fetal heart monitoring,etc.Meanwhile,control group was given labeolol tablet orally,100mg/time,3 times a day.Observation group was given labeolol tablet,combined with enoxaparin,2cm periumbilical subcutaneous injection,4000IU/time,twice/day,5 days of treatment,2d withdrawal,circulation administration.There were no differences in blood pressure index,24h urinary protein level and adverse pregnancy outcome between the two groups before and after treatment.Results After treatment,the levels of systolic blood pressure,diastolic blood pressure and 24h urinary protein in observation group were significantly lower than those in control group(P<0.05);compared with the control group,the incidence of preterm labor,fetal distress and postpartum hemorrhage in the observation group was lower(P<0.05).Conclusions Labeolol plus enoxaparin in patients with gestational hyperemia smoothly reduced blood pressure and reduced the risk of adverse pregnancy outcomes.
