基于双心医学模式的社区干预对老年冠心病患者的影响分析

摘　　要：目的探讨基于双心医学模式的社区干预在老年冠心病患者中的应用效果。方法纳入2019年3月-2020年3月于北京市昌平区回龙观社区卫生服务中心就诊的160例老年冠心病患者,采用便利抽签法分为观察组(80例)和对照组(80例)。两组分别采用基于双心医学模式的社区干预治疗和常规社区干预模式治疗。比较两组患者焦虑自评量表(SAS)和抑郁自评量表(SDS)评分、就医满意度量表评分、主要不良心血管事件发生率等情况差异。结果干预前两组患者SAS和SDS评分比较,差异均无统计学意义(P>0.05),干预后观察组SAS和SDS评分低于对照组,差异有统计学意义(P<0.05)。观察组患者对就医满意度量表中的7个项目的满意度高于对照组,观察组主要不良心血管事件发生率低于对照组,差异均有统计学意义(P<0.05)。结论基于双心医学模式的社区干预有利于调整老年冠心病患者的不良情绪,减少不良心血管事件,提高患者就医满意度。Objective To investigate the application effects of the community intervention on the elderly patients with coronary heart disease based on psychological and cardiological medical model.Methods Totally 160 elective elderly patients with coronary heart disease from March 2019 to March 2020 in our hospital,were randomly divided into the observation group and control group,80 cases in each group.The observation group received community intervention based on psychological and cardiological medical model,and the control group received the routine community intervention model.The differences of the scores of self rating anxiety scale(SAS)and self rating depression scale(SDS),the score of patients'satisfaction scale,and major adverse cardiovascular events were compared in two groups.Results There were no statistical significances in SAS and SDS scores between the two groups before intervention(P>0.05).After intervention,the SAS and SDS scores in the observation group were lower than those of the control group,and the differences were statistical significant(P<0.05).Seven items of patients'satisfaction scale in the observation group were higher than those of the control group,and the incidence of major adverse cardiovascular events(MACE)in the observation group was lower than that of the control group,there were statistical significances in the above items(P<0.05).Conclusion The community interverntion based on psychological and cardiological medical model is helpful to regulate the negative emotions of elderly patients with coronary heart disease,reduce the adverse cardiovascular events,and improve patients'medical satisfaction.
