摘 要:目的探索县域医共体模式下县区级龙头医院如何可持续发展,并提出相应对策。方法借用案例分析法,分析县域医共体的建设现状与存在问题。结果区域内的龙头医院能够充分发挥医保政策的调控作用,但存在优势学科提升缓慢,上下级医院转诊通道不通畅;医保支付方式的改革未触及核心和龙头医院自身内生发展动力缺乏等问题。结论县域医共体建设中,龙头医院要加强专科建设,不断提升自身服务能力;深化医保支付方式改革,充分发挥医保的调控作用;制订规范化的转诊流程,畅通双向转诊通道;健全绩效考核机制,提高各方参与协作的积极性。Objective To explore the sustainable development of leading hospitals in county medical community,and put forward corresponding countermeasures.Methods The case analysis method was used to analyze the construction status and existing problems of county medical community.Results The leading hospital in the region,gives full play to the regulatory role of medical insurance policies.However,the leading hospital in the county has the problems of slow discipline improvement of dominant subjects,and the obstructed referral channel of upper and lower levels of hospitals.The reform of medical insurance payment mode has not touched the core problems,and the lack of internal development power of leading hospitals.Conclusion In the construction of county medical community,leading hospitals should strengthen the construction of specialties and constantly improve their service capacity.The reform of medical insurance payment methods should be deepened to give full play to the regulatory role of medical insurance.A standardized referral process should be established to smooth two-way referral channels.The performance appraisal mechanism should be improve to increase the enthusiasm of all parties to participate in cooperation.
