选择性痔上黏膜大C环式切除术联合中药内服治疗直肠前突的效果

摘　　要：目的:探讨选择性痔上黏膜大C环式切除术联合中药内服治疗直肠前突的效果。方法:选取2016年1月—2022年8月龙岩市第二医院收治的66例直肠前突患者作为研究对象。根据随机数表法将其分为观察组(n=33)和对照组(n=33)。对照组给予选择性痔上黏膜大C环式切除术,观察组给予选择性痔上黏膜大C环式切除术联合中药内服。比较两组术前、术后1周、1个月、2个月排便情况,临床疗效,术前、术后2个月肛门坠胀程度积分,并发症。结果:术后1周、1个月、2个月,两组排便障碍综合征(obstructed defecation syndrome,ODS)评分降低,且观察组ODS评分均低于对照组,差异有统计学意义(P<0.05)。观察组总有效率高于对照组,差异有统计学意义(P<0.05)。术后2个月,观察组肛门坠胀程度积分低于对照组,差异有统计学意义(P<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:选择性痔上黏膜大C环式切除术联合中药内服治疗直肠前突有助于帮助患者改善排便障碍情况,减轻肛门坠胀程度,治疗直肠前突效果肯定。Objective:To explore effect of selective large C-ring resection of upper hemorrhoidal mucosa combined with traditional Chinese medicine oral administration in the treatment of rectocele.Method:A total of 66 patients with rectocele treated in the Second Hospital of Longyan from January 2016 to August 2022 were selected as the study objects.They were divided into observation group(n=33)and control group(n=33)according to random number table method.The control group was given selective large C-ring resection of upper hemorrhoidal mucosa,and the observation group was given selective large C-ring resection of upper hemorrhoidal mucosa combined with traditional Chinese medicine oral administration.The defecation situation preoperative,postoperative 1 week,1 month,2 months,clinical efficacy,the degree of anal distention preoperative and preoperative 2 months and complications were compared between the two groups.Result:At 1 week,1 month,and 2 months after surgery,the score of obstructed defecation syndrome(ODS)was decreased in both groups,and the ODS scores in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).At 2 months after surgery,the score of the degree of anal distention in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:Selective large C-ring resection of upper hemorrhoidal mucosa combined with traditional Chinese medicine oral administration in the treatment of rectocele can help patients to improve defecation obstruction situation,reduce the degree of anal distention,and the effect in the treatment of rectocele is positive.
