丁丙诺啡贴剂联合自控式镇痛在骨恶性肿瘤术后镇痛临床效果研究

摘　　要：目的评价丁丙诺啡贴剂联合自控式镇痛泵(PCIA)对骨恶性肿瘤患者术后镇痛的安全性与效果。方法选取患者60例,随机分成丁丙诺啡贴剂联合PCIA组(A)和舒芬太尼联合PCIA组(B),每组各30例患者。记录术后第0、3、7、14、21、28天的记录NRS评分、使用PCIA次数、不良反应的发生率及补充药物的人数。同时记录患者年龄、体重、血压和心率等一般信息。用SPSS 22.0软件对患者资料进行分析,评价相关指标。结果两组患者一般情况无显著性差异(P≥0.05)。28 d内A组的PCIA次数及补充药物的人数均小于B组,表明差异有显著性(P<0.05)。在术后3 d时,两组采用数字评分法进行比较,具有统计学意义。60例骨恶性肿瘤患者治疗后出现轻微不良反应,但都未发生严重副作用;A、B组不良反应的发生率分别为8.3%、11.7%。结论丁丙诺啡贴剂联合PCIA应用于骨恶性肿瘤术后镇痛具有效果确切、不良反应少等特点,患者总体满意度高,为将来其疗效进一步研究提供参考依据。Objective To evaluate the safety and effect of buprenorphine patch combined with Patientcontrolled Intravenous Analgesia(PCIA)on postoperative analgesia in patients with bone malignant tumors.Methods Sixty patients were selected and randomly divided into buprenorphine patch combined with PCIA group(A)and sufentanil combined with PCIA group(B),with 30 patients in each group.The numeric rating scale(NRS),the PCIA pressing times,the incidence of adverse reactions and the number of supplementary drugs on the 0,3,7,14,21,a nd 28 days after the operation were recorded.At the same time,general information such as the patient's age,weight,blood pressure and heart rate were recorded.The SPSS 22.0 software was used to analyze the patient data and evaluate related indicators.Results Within 28 days,the PCIA pressing times and the number of supplementary drugs in group A were significantly lower than those in group B,indicating that the difference was significant(P<0.05),whereas there was no significant difference in general conditions between the two groups of patients(P≥0.05).Three days after the operation,the two groups were compared with a numerical scoring method,which was statistically significant.Sixty patients with bone malignant tumors had mild adverse reactions after treatment,but none of them had serious adverse reactions;the incidence of adverse reactions in groups A and B were 8.3%and 11.7%,respectively.Conclusion The application of buprenorphine patch combined with PCIA for postoperative analgesia of bone malignant tumors has the characteristics of definite effect,f ew adverse reactions,and high overall patient satisfaction,which provides a reference for further research on its efficacy in the future.
