李全智辨治青光眼大泡性角膜病变的思路及经验

摘　　要：文章主要总结李全智治疗青光眼大泡性角膜病变的思路及经验。李全智根据审证求因,溯本求源,明确病机的总体思路,将青光眼大泡性角膜病变分为郁热犯目,黑睛生泡证;余火未尽,黑睛生泡证;血虚失养,黑睛生泡证三种证候。郁热上犯,内郁不宣,宜清热散邪,通腑明目,以升降散为主方加减;余火未尽,黑睛生泡,宜清肝息风,消泡明目,用沈氏息风汤加减;血虚失养,病情迁延,宜滋养营血,止痛消泡,用托里定痛汤加减。临证灵活施治,疗效可期。This article provides a summary and introduction to the approach and experience of Professor LI Quanzhi in the treatment of bullous corneal lesions in glaucoma.Based on the principles of evidence-based medicine and seeking the root causes,LI Quanzhi has developed a comprehensive approach to understanding the pathogenesis of the disease.The bullous corneal lesions in glaucoma are classified into three types:Congestion-heat attacking the eyes and causing blistering of the cornea,residual fire not fully extinguished causing blistering of the cornea,and blood deficiency leading to malnourishment and blistering of the cornea.For congestion-heat attacking the eyes,where internal stagnation is present,the main treatment approach is to clear heat,dispel pathogenic factors,and improve bowel movement and visual acuity using ascending and descending dispersing prescriptions with appropriate modifications.For residual fire not fully extinguished causing blistering of the cornea,the treatment approach is to clear liver heat,suppress wind,and resolve blisters while improving visual acuity using Shen´s Xifeng Soup decoction with appropriate modifications.For blood deficiency leading to malnourishment and blistering of the cornea,where the disease persists,the treatment approach is to nourish and replenish blood,relieve pain,and resolve blisters using Tuoli Dingtong Decoction with appropriate modifications.With flexible and individualized treatment strategies,promising therapeutic effects can be expected.
