归蛭温胆汤联合醒脑开窍针法对脑卒中后偏瘫患者下肢运动功能及步行能力的影响

摘　　要：目的 探讨归蛭温胆汤联合醒脑开窍针法对脑卒中后偏瘫患者下肢运动功能及步行能力的影响。方法 选取2021年7月至2022年7月湖南中医药大学第二附属医院针灸推拿康复科收治的96例脑卒中后偏瘫患者作为研究对象,按治疗方法不同分为联合治疗组(n=48)与单一治疗组(n=48)。单一治疗组采用醒脑开窍针法治疗,联合治疗组采用归蛭温胆汤联合醒脑开窍针法治疗。比较两组患者下肢运动功能、步行能力及中医证候积分。结果 治疗前,两组简式Fugl-Meyer运动功能量表(FAM)以及功能性步行能力量表(FAC)评分比较差异无统计学意义(P>0.05);治疗后,联合治疗组FAM、FAC评分高于单一治疗组(P<0.05);治疗前,两组半身不遂、肢体麻木、头晕目眩积分比较差异无统计学意义(P>0.05);治疗后,联合治疗组半身不遂、肢体麻木、头晕目眩积分均低于单一治疗组(P<0.05)。结论 采用归蛭温胆汤联合醒脑开窍针法治疗脑卒中后偏瘫患者可提高其下肢运动功能及步行能力,改善临床症状。Objective To explore the effects of Guizhu Wendan Decoction combined with Xingnaokaiqiao acupuncture on lower limb motor function and walking ability of patients with hemiplegia after stroke.Methods From July 2021 to July 2022,96patients with post-stroke hemiplegia who were admitted to the Acupuncture and Moxibustion Massage Rehabilitation Department of the Second Affiliated Hospital of Hunan University of Traditional Chinese Medicine were selected as the research objects.According to different treatment methods,they were divided into the combined treatment group(n=48) and the single treatment group(n=48).The single treatment group was treated with Xingnao Kaiqiao acupuncture,and the combined treatment group was treated with Guizhu Wendan Decoction combined with Xingnao Kaiqiao acupuncture.Lower limb motor function,walking ability and TCM syndrome scores were compared between the two groups.Results Before treatment,there was no statistically significant difference between the two groups in the scores of the Simplified Fugl Meyer Motor Function Scale(FAM)and the Functional Walking Ability Scale(FAC)(P>0.05);After treatment,the FAM and FAC scores in the combined treatment group were higher than those in the single treatment group(P<0.05);Before treatment,there was no statistically significant difference in the scores of hemiplegia,limb numbness,and dizziness between the two groups(P>0.05);After treatment,the scores of hemiplegia,limb numbness,dizziness and dizziness in the combined treatment group were lower than those in the single treatment group(P<0.05).Conclusion Using Guizhi Wendan Tang combined with Xingnao Kaiqiao Acupuncture to treat hemiplegia patients after stroke can improve their lower limb motor function and walking ability,and improve clinical symptoms.
