摘　　要：目的:研究乳腺原发性鳞状细胞癌进行根治性手术加或不加辅助放疗的结果。方法:从SEER数据库中获取组织学诊断为乳腺原发性鳞状细胞癌的患者数据。采用竞争风险模型、Kaplan-Meier方法和Lasso Cox回归模型评估有或无辅助放疗的手术类型对病因特异性生存率和总生存率的影响。结果:本研究共纳入454例乳腺原发性鳞状细胞癌患者。构建多因素竞争风险模型分析发现,接受单独乳腺切除术治疗的患者比接受肿瘤切除术加辅助放疗的患者死于癌症的风险更高(HR 1.77,p<0.05)。根据Kaplan-Meier分析,接受肿瘤切除术加辅助放疗的患者比接受肿瘤切除术、乳腺切除术和乳腺切除术加辅助放疗的患者生存率更高。结论:肿瘤切除术加辅助放疗显著提高了生存率,可能是乳腺原发性鳞状细胞癌的一种治疗选择。Objective:To study the results of radical surgery with or without adjuvant radiotherapy for primary squamous cell carcinoma of the breast.Methods:Data of patients were obtained from the SEER database,which was diagnosed histologically primary squamous cell carcinoma of the breast.The competing hazards model,Kaplan-Meier method and Lasso Cox regression model were used to evaluate the effect of surgical type with or without adjuvant radiotherapy on cause-specific survival and overall survival.Results:A total of 454 patients with primary breast squamous cell carcinoma were included in this study.The multivariate competitive risk model analysis showed that patients receiving mastectomy alone had a higher risk of dying of cancer than patients receiving tumor resection plus adjuvant radiotherapy(HR 1.77,p<0.05).According to Kaplan Meier analysis,the survival rate of patients receiving tumor resection plus adjuvant radiotherapy is higher than that of patients receiving tumor resection,mastectomy and mastectomy plus adjuvant radiotherapy.Conclusion:Tumor resection plus adjuvant radiotherapy significantly improves the survival rate,which may be a treatment option for primary breast squamous cell carcinoma.
