痰菌阴性肺结核电子支气管镜检查的诊断价值研究

摘　　要：探讨了痰菌阴性肺结核电子支气管镜检查的诊断价值。选择贵州省六盘水市六枝特区人民医院2020年3月—2023年4月收治的54例疑似痰菌阴性肺结核者为研究样本,所有研究对象均实施经电子支气管镜肺泡灌洗术、经电子支气管镜黏膜刷检术以及联合方式进行诊断,以涂片抗酸杆菌阳性或结核分枝杆菌利福平耐药检测阳性诊断结果为金标准,以明确诊断结果与诊断效能情况。54例疑似痰菌阴性肺结核患者确诊48例,其中疾病类型以及数量包括溃疡型11例、炎性浸润型7例、疤痕狭窄型10、干酪坏死型12、增殖型8例。电子支气管镜黏膜刷检术阳性检出20例,电子支气管镜肺泡灌洗术阳性检出45例,联合检查阳性检出48例。研究发现,电子支气管镜检查过程中将经电子支气管镜肺泡灌洗术,经电子支气管镜黏膜刷检术联合使用在痰菌阴性肺结核检查中,能够从整体上提升疾病阳性检出情况。The diagnostic value of electronic Bronchoscopy in sputum negative pulmonary tuberculosis was discussed.54 cases of suspected sputum negative pulmonary tuberculosis admitted to Liuzhi Special District People’s Hospital,Liupanshui City,Guizhou Province from March 2020 to April 2023 were selected as the study samples.All of them were diagnosed by electronic bronchoscope alveolar lavage,electronic bronchoscope mucosal brushing,and a combination of methods.The gold standard was the positive diagnosis results of acid fast bacteria smear or Rifampicin resistance test of Mycobacterium tuberculosis,To clarify the diagnostic results and diagnostic effectiveness.54 suspected sputum negative pulmonary tuberculosis patients were diagnosed in 48 cases,including 11 cases of ulcer type,7 cases of infl ammatory infi ltration type,10 cases of scar stenosis type,12 cases of cheese necrosis type,and 8 cases of proliferative type.20 cases were positive for electronic bronchoscopy mucosal brushing,45 cases were positive for electronic bronchoscopy alveolar lavage,and 48 cases were positive for combined examination.The study found that the combination of electronic Bronchoscopy alveolar lavage and electronic bronchoscope mucosal brushing in sputum negative pulmonary tuberculosis examination can improve the positive detection of disease as a whole.
