植入左心室辅助装置术后患者生存体验的质性研究

摘　　要：了解植入左心室辅助装置术后患者的生存体验,为该类患者提供针对性的帮助,为改善临床护理工作提供参考。按照现象学研究方法,于2022年5月—2023年3月采用目的抽样法,选择南京医科大学附属南京医院心脏外科7例植入左心室辅助装置术后存活的患者就生存体验进行半结构式访谈,应用Colaizzi七步分析法对访谈结果进行整理和分析。结果显示,植入左心室辅助装置术后患者生存体验可以归纳为两个主题,分别为机体生理功能的变化(活动能力的改善,睡眠障碍,饮食的改善或障碍)、情感与认知的改变(感恩与乐观生活,多维负性情绪,社会及家庭需求)。研究发现,护理工作者应当重视并关注植入左心室辅助装置术后患者生理功能的改变与心理需求,了解植入左心室辅助装置后患者的生存质量,给予专业支持,对患者进行针对性指导。Understand the survival experience of patients after implantation of left ventricular assist devices,provide targeted assistance for such patients,and provide reference for improving clinical nursing work.Using phenomenological research methods,from May 2022 to March 2023,using the purposive sampling method,seven patients who survived the implantation of left ventricular assist devices in Cardiac surgery of Nanjing First Hospital,Nanjing Medical University were selected for semi structured interview on their survival experience.Colaizzi’s seven step analysis method was used to collate and analyze the interview results.The results show that the survival experience of patients after implantation of left ventricular assist device can be summarized into two themes,namely,changes in physiological functions of the body(improvement of activity ability,Sleep disorder,improvement or obstacle of diet),changes in emotion and cognition(gratitude and optimistic life,multi-dimensional negative emotions,social and family needs).The research results show that nursing workers should pay attention to and pay attention to the changes in physiological function and psychological needs of patients after implantation of left ventricular assist devices,provide professional and emotional support,understand the quality of life of patients with implantation of left ventricular assist devices,and provide targeted guidance to patients.
