摘　　要：目的探讨自拟前列疏解汤联合盐酸坦索罗辛治疗慢性前列腺炎(CP)患者的疗效及其对勃起功能、慢性前列腺炎症状(NIH-CPSI)评分的影响。方法选取2019年1月至2022年12月芜湖市中医医院收治的102湿热下注型CP患者作为研究对象。采用随机数字表法将其分为A组和B组,每组51例。A组采用自拟前列疏解汤联合盐酸坦索罗辛、左氧氟沙星治疗,B组采用盐酸坦索罗辛、左氧氟沙星治疗,均连续治疗4周。比较两组临床疗效和治疗前后NIH-CPSI、国际勃起功能指数-5(IIEF-5)、勃起硬度分级量表(EHS)评分、前列腺液(EPS)中卵磷脂小体(SPL)计数、白细胞计数(WBC)、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平及治疗期间不良反应发生率。结果A组总有效率显著高于B组(P<0.05);治疗后,A组NIH-CPSI评分、SPL计数高于B组,IIEF-5、EHS评分和WBC、IL-1β、TNF-α水平低于B组(P<0.05);两组治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论自拟前列疏解汤联合盐酸坦索罗辛可提高湿热下注型CP临床疗效,改善患者勃起功能,提高NIH-CPSI评分。Objective To explore the curative effect of self-made Qianlie Shujie decoction combined with tamsulosin hydrochloride on patients with chronic prostatitis(CP)and its influences on scores of erectile function and national institutes of health chronic prostatitis symptom index(NIH-CPSI).Methods A total of 102 patients with CP of dampness-heat diffusing downward syndrome admitted to Wuhu Hospital of Traditional Chinese Medicine were enrolled between January 2019 and December 2022 as the research objects.According to random number table method,they were divided into group A and group B,51 cases in each group.The group A was treated with self-made Qianlie Shujie decoction combined with tamsulosin hydrochloride and levof loxacin,while group B was only treated with tamsulosin hydrochloride combined with levof loxacin.Both groups were treated for 4 weeks.The clinical curative effect,scores of NIH-CPSI,international index of erectile function-5(IIEF-5),erection hardness grading scale(EHS),counts of secretions lecithin corpuscles(SPL),white blood cell(WBC)in expressed prostatic secretions(EPS),levels of interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)before and after treatment,and incidence of adverse reactions during treatment were compared between the two groups.Results The response rate of group A was significantly higher than that of group B(P<0.05).After treatment,NIH-CPSI score and SPL count in group A were higher than those in group B,while IIEF-5 and EHS scores,WBC count,IL-1βand TNF-αlevels were lower than those in group B(P<0.05).There was no significant difference in incidence of adverse reactions between group A and group B(P>0.05).Conclusions Self-made Qianlie Shujie decoction combined with tamsulosin hydrochloride can improve clinical curative effect,erectile fu nction and NIH-CPSI score in patients with CP of dampness-heat diffusing downward syndrome.
