益肾柔肝法联合屈螺酮炔雌醇用于体重正常型多囊卵巢综合征患者的临床疗效研究

摘　　要：目的:观察益肾柔肝方药联合屈螺酮炔雌醇对体重正常型多囊卵巢综合征(PCOS)的治疗效果。方法:选取2020年6月至2022年1月该院收治的体重正常型PCOS患者90例,根据随机数字表法分为西药组、中药组和中药+西药组,各30例。西药组患者口服屈螺酮炔雌醇,中药组患者口服自拟补肾柔肝方,中药+西药组患者同时口服治屈螺酮炔雌醇和自拟补肾柔肝方。持续治疗3个月后,评估三组患者的临床有效率,对比三组患者治疗前后中医证候评分变化,血清性激素[卵泡刺激素(FSH)、黄体生成素(LH)和抗米勒管激素(AMH)]水平,脂代谢指标[高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和总胆固醇(TC)]及胰岛素抵抗指标[空腹血糖(FBG)、空腹胰岛素(FINS)和胰岛素抵抗指数(HOMA-IR)]水平变化。结果:治疗3个月后,西药组、中药组和中药+西药组患者的临床总有效率分别为60.0%(18/30)、76.7%(23/30)和93.3%(28/30),中药+西药组患者临床总有效率明显高于西药组、中药组,中药组患者的临床总有效率明显高于西药组,差异均有统计学意义(P<0.05)。治疗3个月后,与中药组、西药组相比,中药+西药组患者的中医证候评分明显降低,血清LH、AMH水平明显降低,FSH水平明显升高,血清LDL-C、TC水平明显降低,FBG、FINS和HOMA-IR水平明显降低,HDL-C水平明显升高,差异均有统计学意义(P<0.05)。治疗3个月后,中药组患者的中医证候评分,血清性激素、脂代谢及胰岛素抵抗指标水平改善情况均优于西药组,差异均有统计学意义(P<0.05)。结论:针对体重正常型PCOS患者的治疗,应用益肾柔肝方药和屈螺酮炔雌醇均可有效减轻患者症状,改善性激素和脂代谢水平,抑制机体胰岛素抵抗,益肾柔肝方药与屈螺酮炔雌醇联合应用的效果较单独应用更佳。OBJECTIVE:To probe into the therapeutic effect of Yishen Rougan formula combined with drospirone and ethinylestradiol on polycystic ovary syndrome(PCOS)with normal weight.METHODS:A total of 90 PCOS patients with normal weight admitted into the hospital from Jun.2020 to Jan.2022 were extracted to divided into the western medicine group,traditional Chinese medicine group,traditional Chinese medicine+western medicine group according to the random number table method,with 30 cases each group.The western medicine group was orally treated with drospirone ethinylestradiol,the traditional Chinese medicine group was orally treated with self-made Bushen Rougan formula,and the traditional Chinese medicine+western medicine group orally received drospirone ethinylestradiol and self-made Bushen Rougan formula.After 3 months of continuous treatment,the clinical effective rate of three groups was evaluated,and changes of traditional Chinese medicine syndrome scores,the levels of serum sex hormones[follicle-stimulating hormone(FSH),luteinizing hormone(LH)and anti-millerian tubular hormone(AMH)],lipid metabolism[high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)and total cholesterol(TC)]and insulin resistance[fasting blood glucose(FBG),fasting insulin(FINS)and insulin resistance index(HOMA-IR)]before and after treatment were compared.RESULTS:After 3 months of treatment,the total clinical effective rates of western medicine group,traditional Chinese medicine group and traditional Chinese medicine+western medicine group were respectively 60.0%(18/30),76.7%(23/30)and 93.3%(28/30).The total clinical effective rate in the traditional Chinese medicine+western medicine group was significantly higher than that in the western medicine group and traditional Chinese medicine group,and the total clinical effective rate in the traditional Chinese medicine group was significantly higher than that in the western medicine group,with statistically significant differences(P<0.05).After 3 months of treatment,compa
