银屑病患者血清和皮肤组织中血管性血友病因子的表达水平及临床意义

摘　　要：目的探讨银屑病患者血清和皮肤组织中血管性血友病因子(vWF)的表达水平及临床意义。方法招募2021年1月至2022年12月吕梁市人民医院收治的寻常型银屑病患者30例(银屑病组),另选择30例皮肤组织正常的外科手术患者作为对照组。采用酶联免疫吸附测定(ELISA)法测定两组受试者血清vWF表达水平;采用免疫组化染色法检测两组皮肤组织vWF表达水平。分析血清vWF表达水平与皮肤组织vWF表达强度及银屑病面积严重指数(PASI)评分的相关性。结果银屑病组血清vWF表达水平高于对照组,差异有统计学意义(P<0.05)。在皮肤的表皮部位以及真皮部位的炎症细胞和内皮细胞中,银屑病组的vWF表达强度较对照组更高,中、重度表达占比更大,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,银屑病患者血清vWF表达水平与PASI评分呈正相关(r=0.432,P=0.009)。单因素方差分析结果显示,银屑病患者血清vWF表达水平与真皮和表皮中vWF的表达强度无显著关联(P>0.05)。结论银屑病患者血清vWF表达水平升高,与PASI评分呈正相关。Objective To investigate the expressions of von Willebrand factor(vWF)in serum and skin tissues of patients with psoriasis and their clinical significance.Methods Thirty patients with psoriasis vulgaris who were admitted to Lvliang People′s Hospital from January 2021 to December 2022 were recruited as the psoriasis group,and other 30 surgical patients with normal skin tissues were selected as the control group.The expression levels of the serum vWF in the subjects of the two groups were determined by enzyme-linked immunosorbent assay(ELISA).The expression levels of vWF in the skin tissues of the two groups were detected by immunohistochemical staining.The correlations of the expression levels of serum vWF with the expression intensity of vWF in skin tissues and the psoriasis area and severity index(PASI)scores were analyzed.Results The expression levels of the serum vWF in the psoriasis group were higher than those in the control group,and the differences were statistically significant(P<0.05).In the epidermis and the inflammatory cells and endothelial cells of the dermis,the expression intensity of vWF in the psoriasis group was higher than that in the control group,and the proportion of moderate and severe expressions in the psoriasis group was greater,and the differences were statistically significant(P<0.05).The results of Pearson correlation analysis showed that the expression levels of the serum vWF in the psoriasis patients were positively correlated with PASI scores(r=0.432;P=0.009).The results of one-way ANOVA analysis showed that there were no significant correlations of the expression levels of the serum vWF in the psoriasis patients with the expression intensities of vWF in the dermis and the epidermis(P>0.05).Conclusion The expression levels of serum vWF in psoriasis patients are increased,and are positively correlated with PASI scores.
