摘  要:目的探讨血清半胱氨酸天冬氨酸蛋白酶3(Caspase-3)、脂蛋白相关磷脂酶A2(Lp-PLA2)水平与突发性耳聋(SD)病情严重程度和预后的关系。方法选取100例SD患者为SD组,根病情严重程度分为轻度听力损失者20例、中度听力损失者43例、重度听力损失者25例、极重度听力损失者12例,根据预后情况分为预后不良组38例、预后良好组62例,另选取同期60例体检健康且听力正常者为对照组。收集SD患者临床资料,采用酶联免疫吸附法检测血清Caspase-3、Lp-PLA2水平。通过Spearman相关法分析SD患者血清Caspase-3、Lp-PLA2水平与PTA的相关性,多因素Logistic回归分析SD患者预后不良的影响因素,受试者工作特征(ROC)曲线分析血清Caspase-3、Lp-PLA2水平对SD患者预后不良的预测价值。结果与对照组比较,SD组血清Caspase-3、Lp-PLA2水平升高(P均<0.05)。轻度、中度、重度、极重度听力损失者血清Caspase-3、Lp-PLA2水平依次升高(P均<0.05)。Spearman相关性分析显示,SD患者血清Caspase-3、Lp-PLA2水平与PTA呈正相关(rs分别为0.816、0.848,P均<0.05)。多因素Logistic回归分析显示,年龄增加、PTA升高、全聋型、Caspase-3升高、Lp-PLA2升高为SD患者预后不良的独立危险因素(P均<0.05)。ROC曲线分析显示,血清Caspase-3、Lp-PLA2水平联合预测SD患者预后不良的曲线下面积大于Caspase-3、Lp-PLA2单独预测(P均<0.05)。结论SD患者血清Caspase-3、Lp-PLA2水平升高与病情严重程度和预后相关,有可能成为SD患者预后的预测指标。Objective To investigate the relationships between serum cysteine-aspartic acid protease-3(Caspase-3)and lipoprotein-associated phospholipase A2(Lp-PLA2)levels and the severity and prognosis of sudden deafness(SD).Methods Totally 100 patients with SD were selected as the SD group.According to the severity of the disease,they were divided into the mild hearing loss group of 20 cases,moderate hearing loss group of 43 cases,severe hearing loss group of 25 cases,and very severe hearing loss group of 12 cases;according to the prognosis,they were divided into the poor prognosis group of 38 cases and good prognosis group of 62 cases;meanwhile,60 cases of healthy and normal hearing in the same period were selected as the control group.Clinical data of SD patients were collected and serum Caspase-3 and Lp-PLA2 levels were measured by ELISA.Spearman correlation was used to analyze the correlations between serum Caspase-3 and Lp-PLA2 levels and PTA in SD patients,multi-factor Logistic regression was used to analyze the factors influencing poor prognosis in SD patients,and receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum Caspase-3 and Lp-PLA2 levels on poor prognosis in SD patients.Results Compared with the control group,serum Caspase-3 and Lp-PLA2 levels increased in the SD group(both P<0.05).Serum Caspase-3 and Lp-PLA2 levels increased in the mild,moderate,severe and very severe hearing loss groups in that order(all P<0.05).Spearman correlation analysis showed that serum Caspase-3 and Lp-PLA2 levels were positively correlated with PTA in SD patients(rs=0.816,0.848,both P<0.05).Multi-factor Logistic regression analysis showed that increasing age,elevated PTA,total deafness phenotype,elevated Caspase-3,and elevated Lp-PLA2 were independent risk factors for poor prognosis in SD patients(all P<0.05).ROC curve analysis showed that the area under the curve of serum Caspase-3 and Lp-PLA2 levels combined in predicting poor prognosis in SD patients was greater than that of Caspase-3 and
