基于脑肠互动探析针灸足三里治疗腹泻型肠易激综合征

摘　　要：腹泻型肠易激综合征是一种常见的功能性胃肠病,针灸作为常用中医疗法通过调节脑肠系统治疗该病具有一定优势。脑肠互动为中枢神经系统与消化系统的交互联系,与中医“神”与脾胃相互调节相契合。“脑”在西医学中是指中枢神经系统,在中医学是指神明之主心脑。脑肠互动失调是腹泻型肠易激综合征的重要发病基础,该病的中医病机可概括为“神”与脾胃作用异常,与心神、脑神密切相关。近年来,脑肠互动理论的提出为足三里从神论治该病提供了理论依据,足三里具有健脾养心宁神、疏肝健脾调神、温肾健脾益髓等作用,与现代医学中足三里调控脑肠系统不谋而合。文章基于脑肠互动理论,从中医“调神”及西医调节脑肠系统角度探析针灸足三里治疗腹泻型肠易激综合征,以期为针灸足三里治疗功能性胃肠病提供理论支持和新思路,进一步拓展针灸足三里的应用范围。Irritable bowel syndrome with predominant diarrhea is a common functional gastrointestinal disorder.Acupuncture and moxibustion,as a common traditional Chinese medicine therapy,has certain advantages in treating the disease by regulating the brain-gut system.Brain-gut interaction is the interaction between the central nervous system and the digestive system,which corresponds to the mutual regulation of the“spirit”and the spleen and stomach in traditional Chinese medicine.“Brain”refers to the central nervous system in Western medicine,and refers to the heart and brain governing the spirit in traditional Chinese medicine.The disorder of brain-gut interaction is an important basis for the pathogenesis of irritable bowel syndrome with predominant diarrhea.The TCM pathogenesis of the disease can be summarized as the disorder of“spirit”and the spleen and stomach,which is closely related to the heart mind and the brain mind.In recent years,the theory of brain-gut interaction has provided a theoretical basis for Zusanli(ST36)to treat the disease from the perspective of spirit.Zusanli(ST36)has the functions of invigorating the spleen,nourishing the heart and calming the spirit,soothing the liver and invigorating the spleen to regulate the spirit,warming the kidney,invigorating the spleen and benefiting the marrow,which coincides with Zusanli(ST36)regulation of the brain-gut system in modern medicine.Based on the brain-gut interaction theory,this paper explores Zusanli(ST36)acupuncture and moxibustion treatment of irritable bowel syndrome with predominant diarrhea from the perspective of“regulating the spirit”of traditional Chinese medicine and regulating the brain-gut system of Western medicine,in order to provide theoretical support and new ideas for acupuncture and moxibustion Zusanli(ST36)treatment of functional gastrointestinal diseases,and further expand the application of acupuncture and moxibustion Zusanli(ST36).
