滋阴补阳消渴健骨汤对2型糖尿病合并骨质疏松症(阴阳两虚证)治疗作用

摘　　要：目的 探讨滋阴补阳消渴健骨汤对2型糖尿病(T2DM)合并骨质疏松症(OP)(阴阳两虚证)的治疗作用。方法 选取90例符合纳排标准的T2DM合并OP患者,采用随机数字表分为2组。对照组实施常规西药降糖、抗OP治疗,研究组在对照组基础上予以滋阴补阳消渴健骨汤治疗,两组均维持治疗3个月。比较治疗前后中医症状评分、糖代谢指标、骨密度、骨代谢指标,降糖和抗OP疗效,治疗期间药物不良反应。结果 两组治疗后主症、次症评分和总分,空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、β-胶原降解产物(β-CTX)、Ⅰ型胶原氨基端延长肽(PINP)水平均下降(P<0.05),且研究组均低于对照组(P<0.05);两组治疗后骨密度、骨特异性碱性磷酸酶(BAP)、护骨素(OPG)水平均升高(P<0.05),且研究组均高于对照组(P<0.05);两组降糖和抗OP的疗效等级分布差异均有统计学意义(P<0.05),且研究组的总有效率均高于对照组(P<0.05);两组治疗期间药物不良反应发生率差异无统计学意义(P>0.05)。结论 对T2DM合并OP(阴阳两虚证)采用滋阴补阳消渴健骨汤辅助西医治疗不仅可减轻症状,改善糖代谢和骨代谢,增加骨密度,还可增强单一西医治疗的疗效,且安全。Objective To explore the therapeutic effect of Ziyin Buyang Xiaoke Jiangu Decoction(滋阴补阳、消渴健骨汤)in type 2 diabetes mellitus(T2DM)combined with osteoporosis(OP).Methods 90 patients with T2DM complicated with OP who met the adoption and exclusion criteria were selected,random number table was used to divide into 2 groups.The control group was treated with conventional Western medicine hypoglycemic and anti OP,while the research group was treated with Ziyin Buyang Xiaoke Jiangu Decoction on the basis of the control group,and both groups were maintained for 3 months.The scores of TCM symptoms before and after treatment,glucose metabolism index,bone mineral density,hypoglycemic and anti OP efficacy before and after treatment,bone metabolism indexes before and after treatment,adverse drug reactions during treatment were compared.Results The main symptom,secondary symptom scores and total scores after treatment,fasting plasma glucose(FPG),2 h postprandial plasma glucose(2 h PG),glycosylated hemoglobin(HbA1c),β-collagen degradation products(β-CTX),type I collagen amino terminal lengthening peptide(PINP)levels of the two groups decreased(P<0.05),of which the research group were lower than those of the control group(P<0.05).After treatment,bone mineral density,bone specific alkaline phosphatase(BAP)and osteoprotegerin(OPG)levels increased in both groups(P<0.05),of which the research group were higher than those of the control group(P<0.05).There were significant differences in the grade distribution of hypoglycemic and anti OP efficacy between the two groups(P<0.05),and the total effective rates of the research group were higher than the control group(P<0.05).There was no significant difference in the incidences of adverse drug reactions during treatment between the two groups(P>0.05).Conclusion Treating T2DM with OP(deficiencies of Yin and Yang syndrome)by Ziyin Buyang Xiaoke Jiangu Decoction can not only reduce symptoms,improve glucose metabolism and bone metabolism,increase bone mineral density,but
