天麻治疗偏头痛药理机制及研究进展

摘　　要：偏头痛是临床常见的原发性头痛,反复发作难以治愈,严重危害患者的生活质量,其发病机制复杂多样,目前较为经典的假说有血管学说、炎症介质学说、神经元学说等。而中医学多认为与肝脾肾等脏腑功能失调、经络失养密切相关。近年来中医药治疗偏头痛发挥独特的作用,其中天麻作为抗偏头痛的高频用药,具有熄风止痉、平抑肝阳、祛风通络的功效,临床上已被广泛应用。文章通过查阅大量国内外相关文献,阐述了偏头痛的发病机制和天麻及其活性成分治疗偏头痛的药理机制,发现天麻在调节血管活性物质、调节神经递质水平、减少炎症介质水平、调控小胶质细胞活性等方面发挥重要作用。文章对天麻在偏头痛治疗方面的积极作用加以总结,为天麻广泛临床应用提供科学依据。Migraine is a common primary headache,which is difficult to cure repeatedly and seriously harms the quality of life of patients.Its pathogenesis is complex and diverse.At present,the more classical hypotheses include vascular theory,inflammatory mediator theory,neuron theory and so on.And the Chinese medicine thinks that it is closely related to the dysfunction of viscera such as liver,spleen and kidney and insufficiency of meridians.In recent years,traditional Chinese medicine has played a unique role in the treatment of migraine,among which Tianma(Gastrodiae Rhizoma),as a high-frequency anti-migraine drug,has the effect of calming wind and spasmodic,calming liver Yang,dispelling wind and removing collaterals,which has been widely used in clinical practice.By reviewing a large number of relevant literatures at home and abroad,this paper expounds the pathogenesis of migraine and the pharmacological mechanism of Tianma(Gastrodiae Rhizoma)and its active components in the treatment of migraine.It is found that Tianma(Gastrodiae Rhizoma)plays an important role in regulating vasoactive substances,regulating the level of neurotransmitters,reducing the level of inflammatory mediators and regulating the activity of microglia.This article summarizes the positive effects of Tianma(Gastrodiae Rhizoma)in the treatment of migraine,and provides scientific basis for the extensive clinical application of Tianma(Gastrodiae Rhizoma).
