摘 要:目的 基于“心脾相关”理论观察经验方健脾益心方对心肌梗死大鼠线粒体功能及炎症反应的改善作用并探讨其可能的机制。方法 结扎SD大鼠冠状动脉左前降支复制心肌梗死模型,健脾益心方给药4周,使用心脏超声心动图观察心脏结构与功能变化。分离心脏组织采用HE染色与Masson染色观察心肌组织病理改变,透射电镜观察心肌线粒体超微机构,免疫组化与Western Blot检测炎症因子指标。结果 (1)前降支结扎4周后,与假手术组比较,模型组左室舒张末期内径(leftventricular end-diastolic internal diameters,LVIDd)、室收缩末期内径(leftventricular end-systolic internal diameters,LVIDs)升高(P<0.01或P<0.000 1),左室射血分数(leftventricular ejection fraction,LVEF)、左室短轴缩短率(leftventricular fractional shortening,LVFS)降低(P<0.000 1),差异具有统计学意义,提示冠状动脉结扎后大鼠心功能损害,左心室扩大、收缩能力降低;与模型组比较,健脾益心方组LVEF、LVFS升高(P<0.001或P<0.000 1),LVIDs降低(P<0.01),差异具有统计学意义。(2)分析心脏形态的变化和心脏重量/体质量比(HW/BW),发现各组大鼠存在心肌重构。模型组心脏外观及HW/BW结果显示心肌轻度增大(P<0.01),健脾益心方干预后心肌状况较模型组改善(P<0.05)。(3)模型组大鼠心肌梗死边界区细胞排列不规则,甚至坏死,炎症浸润,纤维瘢痕形成(P<0.001),健脾益心方组可不同程度地改善心肌组织病理损伤,抑制心肌纤维化改变(P<0.05)。(4)模型组大鼠心肌细胞排列不规整,心肌细胞间存在大量的炎症细胞、成纤维细胞。线粒体聚集成堆,排列不规则且线粒体变形。健脾益心方可明显改善心肌线粒体结构。(5)与假手术组比较,模型组大鼠NLRP3、NF-κB p65、cleaved-IL-1β和IL-1β的蛋白表达量明显增加(P<0.05),差异具有统计学意义;与模型组比较,健脾益心方组NLRP3、NF-κB p65、cleaved-IL-1β和IL-1β蛋Objective To observe the effect of Jianpi Yixin Decoction(健脾益心方)on mitochondrial function and inflammatory response in rats with myocardial infarction based on the theory of“heart-spleen correlation”and to explore its possible mechanism.Methods The myocardial infarction model was established by ligation of the left anterior descending coronary artery in SD rats.The cardiac structure and function changes were observed by echocardiography after 4 weeks of Jianpi Yixin Decoction administration.HE and Masson staining were used to observe the pathological changes of cardiac tissue.Transmission electron microscopy was used to observe the ultrastructure of myocardial mitochondria.Immunohistochemistry and Western Blot were used to detect the related proteins and inflammatory indicators.Results(1)After 4 weeks of anterior descending branch ligation,compared with the sham surgery group,the model group showed an increase in left ventricular end diastolic internal diameter(LVIDd)and left ventricular end systolic internal diameter(LVIDs)(P<0.01 or P<0.0001),as well as an increase in left ventricular ejection fraction(LVEF)The left ventricular fractional shortening(LVFS)rate decreased(P<0.0001)with statistical significance,indicating that after coronary artery ligation in rats,cardiac function was impaired,left ventricular enlargement and contraction ability were reduced;Compared with the model group,Jianpi Yixin Decoction group showed an increase in LVEF and LVFS(P<0.001 or P<0.0001),and a decrease in LVIDs(P<0.01),with statistically significant differences.(2)Analyzing the changes in cardiac morphology and the heart weight to body weight ratio(HW/BW),it was found that there was myocardial remodeling in each group of rats.The model group's cardiac appearance and HW/BW results showed mild myocardial enlargement(P<0.01),and the myocardial condition improved after the intervention of Jianpi Yixin Decoction compared to the model group(P<0.05).(3)The model group rats had irregular arrangement of cells in the boundary
