摘　　要：糖尿病肾病是由糖尿病引起的慢性肾脏疾病,现已成为终末期肾病的首要病因,严重影响患者的生活质量,并加重社会经济负担。该病发病机制复杂,尚缺乏特效疗法,目前主要采用综合治疗手段,中医药在辨证论治的基础上能够帮助患者延缓疾病进展,是极具潜力的治疗措施。张兰教授根据多年临床实践,结合中医玄府理论,提出“肾玄府开阖不利”导致糖尿病肾病(diabetic kidney disease,DKD)的微观病机,治疗时在辨证基础上,注重通利玄府,取得了较好的疗效。该文基于玄府理论视角,系统阐述玄府的结构、生理功能、病理变化与现代医学肾脏的关联,肾玄府开阖不利导致糖尿病肾病(DKD)的病机,以及张兰教授应用通利玄府法治疗DKD的经验。Diabetic kidney disease(DKD)is a chronic kidney disease caused by diabetes.It has become the primary cause of end-stage renal disease,which seriously affects the quality of life of patients and increases the social and economic burden.The pathogenesis is complex,and there is still lack of specific treatment.At present,comprehensive treatment is mainly used.Traditional Chinese medicine can help patients delay the progress of the disease on the basis of syndrome differentiation,which is a treatment measure with great potential.Professor ZHANG Lan,based on years of clinical practice,combined with the theory of Xuanfu in traditional Chinese medicine,proposed the micro pathogenesis of diabetes nephropathy caused by“he opening and closing of Xuanfu is not favorable”.On the basis of syndrome differentiation,he paid attention to the facilitation of Xuanfu,and achieved good results.Based on the perspective of Xuanfu theory,this paper systematically expounds the relationship between the structure,physiological function,patho logical changes of Xuanfu and modern medical kidneys,the pathogenesis of DKD caused by the unfavorable opening and closing of Xuanfu,and Professor ZHANG Lan's experience in treating DKD with Tongli Xuanfu method.
