基于脾气散精理论探讨血糖稳态调节

摘　　要：糖尿病患者由于胰岛素敏感性降低,维持血糖稳态的功能下降,而出现波动性高血糖,脾运化功能失调,不能将葡萄糖这种精微物质转输布散到全身,使葡萄糖在血中堆积发生波动性高血糖。研究表明血糖波动是糖尿病慢性并发症的重要预测因素,血糖波动对机体产生的影响和损害明显高于单纯高血糖状态。糖尿病归属中医“消渴”范畴,其病因病机与脾有着极为密切的关系,如《灵枢·本脏》曰:“脾脆则善病消瘅易伤”。脾不散“精”(血糖),精郁为浊,浊毒壅遏,是血糖稳态失衡的必有环节,而脾虚不能促进胰脏分泌胰岛素从而导致血糖波动,进而形成糖尿病。中药复方益糖康基于此理论运用助脾散精、健脾益气等方法辨证施治,通过促进胰脏分泌胰岛素从而维持血糖稳态,降低糖尿病对机体的损害。Diabetic patients suffer from fluctuating hyperglycemia due to the decrease of insulin sensitivity and the function of maintaining blood glucose homeostasis,and the spleen transport and transformation function is out of order.Glucose,a subtle substance,can not be transported to the whole body,and it can also accumulate in the blood to cause fluctuating hyperglycemia.Studies have shown that blood sugar fluctuation is an important predictor of chronic complications of diabetes,and the impact and damage caused by blood sugar fluctuation on the body is significantly higher than that caused by simple hyperglycemia.Diabetes belongs to the category of quenching thirst in traditional Chinese medicine,and its etiology and pathogenesis are closely related to the spleen.For example,in Miraculous Pivot·Benzang,it is said that“the spleen is brittle,so it is good for the disease and easy to hurt”.Spleen does not disperse essence(blood sugar),which is turbid and turbid,which is an essential link of blood sugar homeostasis.Spleen deficiency can not promote insulin secretion in pancreas,which leads to blood sugar fluctuation,thus forming diabetes.Based on this theory,Yitangkang(益糖康),a traditional Chinese medicine compound,uses the methods of invigorating spleen and dispersing essence,invigorating spleen and benefiting Qi to treat syndrome differentiation,which can maintain blood sugar homeostasis by promoting insulin secretion in pancreas and reduce the damage of diabetes to the body.
