论“态靶因果”辨治在非特异性腰痛中的应用

摘　　要：通过分析仝小林院士“态靶因果”辨治方略内涵,基于中医治疗腰痛的临床实践,探索将该辨治方略应用于非特异性腰痛(nonspecific low back pain, NLBP)的诊疗,以提高临床疗效。参照中医对NLBP原有认识,抓住核心病机,提出该病演变为瘀滞态→虚损态,不同中医态包括不同证型,据此初步确立治法方药。针对该类疾患中医传统病因及现代病因如退变、肌肉骶髂关节因素、病毒感染、情志因素,对症治疗并参照中药现代药理学成果选药“打靶”,提高治疗的精确性。治疗中掌握该病的气血虚实变化,先安未受邪之地,阻止传变,为NLBP的中西医结合治疗提供新思路。By analyzing the connotation of academician TONG Xiaolin's syndrome differentiation and treatment strategy of“state-target-cause-result”,based on the clinical practice of traditional Chinese medicine in the treatment of low back pain,this paper explored the application of this syndrome differentiation and treatment strategy in the diagnosis and treatment of nonspecific low back pain(NLBP)to improve the clinical efficacy.Referring to the original understanding of NLBP in traditional Chinese medicine,grasping the core pathogenesis,it is proposed that the disease evolves from stagnant state to deficient state,and different TCM states include different syndrome types,based on which the treatment methods and prescriptions are preliminarily established.In view of the traditional Chinese medicine etiology and modern etiology of such diseases,such as degeneration,sacroiliac joint factors,viral infection,emotional factors,symptomatic treatment and referring to the modern pharmacological achievements of traditional Chinese medicine,drug selection“targeting”can improve the accuracy of treatment.In the treatment,we can grasp the changes of Qi and blood of the disease,protect the place that has not been affected by the disease and stop the transmission of changes,so as to provide new ideas for the combined treatment of NLBP.
