从“戒在大痈也”探讨糖尿病合并疖痈病机及治疗经验

摘　　要：糖尿病合并疖痈在祖国医学中可归属于“消渴”“疖痈”范畴,病因病机较为复杂,总体可责之于气血两虚,热毒内郁。文章以王焘《外台秘要》“戒在大痈也”为理论基础,对糖尿病合并疖痈病机及治疗方案进行揆度,提出“戒在大痈也”这一理论贯穿于消渴病预后整个病程阶段。该病从病程来看,可分为初期,脓成期和脓溃期,初期病机关键责之于燥热内虚,郁热内结;脓成期责之于正气亏虚,毒邪入络,内外相搏;脓溃期则以气血两亏为主。从“戒在大痈也”理论出发,未病先防,平衡阴阳,消渴日久则积极益气养血,扶助正气,增强机体托毒外出的能力是临床预防和治疗糖尿病合并疖痈的关键。Diabetes mellitus complicated with furuncle can be classified as“thirst quenching”and“furuncle”in Chinese medicine.The etiology and pathogenesis are more complex,and the overall blame can be attributed to deficiency of Qi and blood,heat toxicity and internal depression.Taking Wang Tao“outside Taiwan secret to”“ring in the great carbuncle”as the theoretical basis,in order to diabetic furuncle carbuncle pathogenesis and treatment of conjecture,think“ring in the great carbuncle”this theory runs through the whole course of the disease stage in the prognosis of collateral.From the point of view of the course of the disease,it can be divided into the early stage,purulent stage and purulent stage.The key pathogenesis of the early stage is the deficiency in the desiccant heat,the stagnation heat in the knot.Purulent period of responsibility for the deficiency of positive Qi,toxic evil into collars,internal and external fighting;Pyurulent stage is mainly Qi and blood two deficiency.Starting from the theory of“stopping the carbuncle”,the key to clinical prevention and treatment of diabetic complicated with furuncle is to prevent the disease before it occurs,balance Yin and Yang,relieve thirst for a long time,actively nourish Qi and blood,support healthy Qi,and en-hance the body’s ability to carry poison out.
