摘 要:目的探讨磁共振成像短时间反转恢复序列(MRI-STIR)检查对髋关节大转子疼痛综合征(GTPS)的评估价值。方法选择45例髋关节GTPS患者作为疼痛组,另选择32例无症状的志愿者作为对照组,回顾性分析两组的髋关节MRI-STIR成像资料,比较两组MRI-STIR扫描的GTPS MRI的征象。结果疼痛组中,38例髋关节MRI-STIR图像出现GTPS阳性改变,7例无明显异常表现;38例GTPS MRI阳性征象均出现股骨大转子处臀中肌/臀小肌水肿的表现,其中25例同时合并出现:8例髂胫束增厚水肿、2例出现股骨头大转子骨髓水肿、15例转子区滑囊积液(其中3例伴发钙化性肌腱炎)。对照组中,27例MRI-STIR扫描显示双侧髋关节未见明显异常,5例出现髋关节股骨大转子区周围少许稍高信号,表现为股骨大转子处臀中肌/臀小肌水肿信号略增高。疼痛组GTPS MRI阳性征象率84.4%高于对照组的15.6%,差异有统计学意义(P<0.05)。结论MRI-STIR检查可以较为准确的为髋关节GTPS的诊断提供影像学证据和支持。Objective To discuss the value of magnetic resonance imaging-short time inversion recovery(MRI-STIR)in evaluation of the greater trochanteric pain syndrome(GTPS).Methods 45 patients with GTPS of the hip were selected as the pain group,and another 32 asymptomatic volunteers were selected as the control group,and the hip MRI imaging data of both groups were retrospectively analyzed.The GTPS MRI signs in MRISTIR were compared between the two groups.Results In the pain group,38 cases showed positive GTPS changes on MRI-STIR images of the hip,and 7 cases showed no obvious abnormalities;all 38 cases with positive GTPS MRI signs showed edema of the gluteus medius/gluteus minimus muscles at the greater trochanter of the femur,of which 25 cases were combined at the same time:8 cases had thickening and edema of the iliotibial tract,2 cases had bone marrow edema of the greater trochanter of the femur,and 15 cases had fluid accumulation in the trochanter region(3 of which were accompanied by calcific tendonitis).In the control group,the MRI-STIR scan of 27 patients showed no obvious abnormality in bilateral hip joints,and 5 patients showed slightly elevated signals around the greater trochanter of the hip joint,manifested as slightly increased signals of edema in the gluteus medius/gluteus minor muscle at the greater trochanter of the femur.The rate of positive GTPS MRI signs was 84.4%in the pain group,which was higher than that of 15.6%in the control group,and the difference was statistically significant(P<0.05).Conclusion MRI-STIR can provide more accurate imaging evidence and support for the diagnosis of GTPS of the hip.
