标准外伤大骨瓣开颅术治疗重型颅脑损伤的效果分析

摘　　要：目的:探讨重型颅脑损伤患者实行标准外伤大骨瓣开颅术治疗的效果。方法:选取2021年9月—2022年9月灌南县第一人民医院收治的30例重型颅脑损伤患者作为研究对象,根据随机数字表法分为对照组与观察组,各15例。对照组实施常规骨瓣开颅术治疗,观察组实施标准外伤大骨瓣开颅术治疗。比较两组颅内压、神经功能、并发症发生率、预后情况。结果:治疗后,观察组颅内压、美国国立卫生研究院卒中量表评分低于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P=0.001)。观察组预后恢复良好率高于对照组,差异有统计学意义(P<0.001)。结论:标准外伤大骨瓣开颅术应用于重型颅脑损伤的治疗效果理想,术中暴露充分,能彻底减压,有效减轻神经功能损伤,预后较好。Objective:To investigate the effect of standard large trauma craniotomy in patients with severe craniocerebral injury.Methods:A total of 30 patients with severe craniocerebral injury diagnosed and treated in Guannan First People′s Hospital from September 2021 to September 2022 were selected as the study subjects.According to the random number table method,the patients were divided into control group and observation group,with 15 cases in each group.The control group was treated with conventional bone flap craniotomy,and the observation group was treated with standard large trauma craniotomy.The intracranial pressure,neurological function,complication rate and prognosis in two groups were compared.Results:After treatment,the intracranial pressure and National Institute of health stroke scale scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.001).The prognosis rate of good recovery in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.001).Conclusion:Standard large trauma craniotomy has an ideal therapeutic effect for severe craniocerebral injury,with sufficient intraoperative exposure and complete decompression,it can effectively relieve neurological injury,and has a good prognosis.
