自制敷料按压带在经皮肾镜术后肾造瘘拔管后患者的应用

摘　　要：目的探讨肾造瘘拔管后自制敷料按压带在经皮肾镜术(PCNL)后患者的应用效果。方法该研究制作的经皮肾镜术后拔除肾造瘘管专用敷料按压带由敷料按压带及充气装置2个部分组成。选取在我院泌尿外科于2021年6月至2022年6月行单侧单通道经皮肾镜碎石术后患者80例为试验对象,采用随机数字表法分组为试验组40例和对照组40例。试验组在拔除肾造瘘管后,造瘘口贴上敷料后采用自制造瘘敷料按压带,取健侧卧位按压1h;对照组在拔除肾造瘘后,采用传统方法贴上敷料,取健侧卧位,患者家属或护理人员按压敷料1 h。比较两组患者拔除肾造瘘管后医护工作量(更换敷料次数、更换衣物次数、更换被褥次数、工作耗时),患者舒适度及漏尿率。结果试验组患者肾造瘘管拔除后更换敷料次数、更换衣物次数、更换被褥次数及工作耗时均少于对照组(P<0.05),试验组患者舒适度高于对照组(P<0.05),试验组患者漏尿率低于对照组(P<0.05)。结论肾造瘘拔管后自制敷料按压带既能防止患者漏尿、减轻患者痛苦,又能解放双手,减少人力物力,减轻医务人员的工作负荷,值得临床进行推广及应用。Objective To investigate the application effect of self-made dressings pressing tape after removing nephrostomy tube in patients with percutaneous nephroscopy(PCNL).Methods The special dressing tape for removing nephrostomy tube after percutaneous nephroscopy was composed of two parts:the dressing belt and the inflator.A total of 80 patients who underwent unilateral single-channel percutaneous nephrolithotripsy from June 2021 to June 2022 in the department of urology of my hospital were conveniently selected as test subjects,and were divided into the experimental group(40 cases)and the control group(40 cases)by using random number table method.In the experimental group,after the nephrostomy tube was removed,a self-made dressings pressing tape was applied after the fistula opening was affixed with the dressings.Take health lateral position press for 1 hour;After nephrostomy was removed,patients in the control group were affixed with the dressings by traditional methods and placed in the healthy lateral decubitus position.Patients'family members or nursing staff pressed the dressing for 1 hour.The workload of medical care(the number of dressing changes,clothing changes,bedclothes changes,and work time),patient comfort and urine leakage rate were compared between the two groups.Results The dressings,clothing,bedding changes and work time of experimental group were less than those of control group(P<0.05),the comfort level of experimental group was higher than that of control group(P<0.05),and the leakage rate of patients in the experimental group was lower than that in the control group(P<0.05).Conclusion After nephrostomy tube removal,selfmade dressings pressing tape can not only prevent urine leakage of patients,relieve patients'pain,but also liberate hands,reduce human and material resources,reduce the workload of medical staff,which is worthy of clinical promotion and application.
