摘 要:目的比较Er∶YAG激光蚀刻与35%磷酸酸蚀对牙釉质表面抗酸性作用的影响,分析Er∶YAG激光蚀刻对牙釉质表面抗酸性的作用。方法正畸患者6例减数拔除的第一前磨牙共24颗,制成研究试样,随机分为Er∶YAG激光蚀刻组(L组)和35%磷酸酸蚀组(A组),每组12颗试样。两组试样的牙釉质平面均分为左右两侧,右侧为对照侧不做处理,左侧分别使用Er∶YAG激光蚀刻(参数为能量250 mJ,频率30 Hz,水量6/8)和35%磷酸酸蚀;然后将所有试样置于37℃的pH 4.2脱矿凝胶中3 d进行酸循环处理后,检测试样牙釉质表面PO_(4)^(3-)含量和牙釉质表面微硬度。结果与对照侧比较,Er∶YAG激光蚀刻后牙釉质表面v1型PO_(4)^(3-)波数峰强度显著增加,半波宽度显著减小,牙釉质表面微硬度显著增大(P<0.001);35%磷酸酸蚀后牙釉质表面v1型PO_(4)^(3-)波数峰强度显著降低,半波宽度显著增大,牙釉质表面微硬度显著降低(P<0.001)。两组间研究侧比较,PO_(4)^(3-)波数峰位移均无明显变化(P>0.05)。结论能量250mJ,频率30Hz,水量6/8的Er∶YAG激光蚀刻有利于增强牙釉质表面抗酸性。Objective To compare the effect of Er:YAG laser etching and phosphate etching on the acid resistance of enamel surface and confirm that Er:YAG laser etching will enhance the acid resistance of enamel.Methods Totally 24 first premolars extracted for orthodontic treatment were collected and prepared on the surface of enamel.They were then randomly divided into two groups:the Er:YAG laser etching group and 35%phosphate etching group.The left side of the premolar was etched with Er:YAG laser(250 mJ/30 Hz/W 6/8)or 35%phosphate,and the right side was given no treatment as the control side.All the samples were placed in demineralized gel(pH 4.2,37℃)for 3 days for acid cycle.Raman spectrometers and microhardness instruments were used to analyze the changes of PO_(4)^(3-)on enamel surface and the microhardness of enamel surface respectively.Results Upon the Er:YAG laser etching,the intensity of vl PO_(4)^(3-) wave number peak was much higher,the width of half wave much smaller and the microhardness of enamel surface much greater in the etching side than in the blank control side(P<0.001).After the 35%phosphoric acid etching,the intensity of v1 PO_(4)^(3-)wave number peak was much lower,the width of half wave much greater and the microhardness of enamel surface much smaller in the etching side than in the blank control side(P<0.001).Conclusions Er:YAG laser etching(250 mJ/30 Hz/W 6/8)will enhance the acid resistance of enamel.
