摘 要:目的分析年龄因素下中老年人脊柱-骨盆参数与腰椎旁肌退变的相关性。方法将194例中老年体检者按照年龄段不同分为A组(40~49岁,59例)、B组(50~59岁,77例)及C组(60~69岁,58例)。比较3组脊柱-骨盆参数及腰椎旁肌退变程度指标,分析3组脊柱-骨盆参数与L_(3~4)、L_(4~5)、L_(5)~S_(1)节段腰椎旁肌横截面积(CSA)/椎体CSA的相关性。结果(1)骨盆倾斜角(PT)、骶骨倾斜角(SS)、腰椎前凸角(LL)、骨盆入射角(PI)、PI与LL的差值(PI-LL)3组比较差异均无统计学意义(P>0.05)。(2)各腰椎节段腰椎旁肌CSA/椎体CSA:随A、B、C组均呈逐步下降趋势(P<0.05);3组组内不同腰椎节段腰椎旁肌CSA/椎体CSA:随L_(3~4)、L_(4~5)、L_(5)~S_(1)均呈逐步下降趋势(P<0.05)。(3)3组PT、SS、LL、PI与各腰椎节段腰椎旁肌CSA/椎体CSA的相关性:PT均无相关(P>0.05),SS、LL、PI大体呈正相关(P<0.01)。(4)PI-LL与各腰椎节段腰椎旁肌CSA/椎体CSA的相关性:A组均无相关(P>0.05);B组L_(3~4)、L_(4~5)均无相关(P>0.05)、L5~S1呈弱负相关(P<0.05);C组L_(3~4)、L_(4~5)均呈中等强度负相关(P<0.05)、L_(5)~S_(1)呈弱负相关(P<0.05)。结论中老年人随年龄增加,腰椎旁肌退变程度逐渐加重,甚至可引起腰椎退行性改变。腰椎旁肌的退变随年龄增长与脊柱-骨盆参数间存在一定的相关性,且此种相关性在60岁以上老年人中表现更为明显,提示老年人腰椎旁肌的退变与腰椎退行性改变受年龄因素影响。Objective To analyze the correlation between spine-pelvis parameters and lumbar paraspinal muscle degeneration in the middle-aged and elderly.Methods The 194 physical examinees were divided into group A(40~49 years old,59 cases),group B(50~59 years old,77 cases)and group C(60~70 years old,58 cases).The spine-pelvis parameters and the indicators of lumbar paraspinal muscle degeneration degree of the three groups were compared.The correlation between spine-pelvis parameters and the lumbar paraspinal muscle cross-sectional area(CSA)/vertebral CSA at L_(3~4),L_(4~5)and L_(5)~S_(1)segments among the 3 groups was analyzed.Results①There were no statistical differences in the pelvic tilt(PT),sacral tilt(SS),lumbar lordosis(LL),pelvic incidence(PI)and the difference value of PI and LL(PI-LL)among three groups(P>0.05).②CSA of lumbar paraspinal muscles/vertebral CSA of each lumbar segment:which was decreased gradually with the age increasing of group A,B and C(P<0.05);CSA of lumbar paravertebral muscles/vertebral CSA at different lumbar segments in the three groups:with L_(3~4),L_(4~5)and L_(5)~S_(1),the data was vertebrae decreased gradually(P<0.05).③The correlation of PT,SS,LL,PI of three groups and CSA of lumbar paraspinal muscles/vertebral CSA of each lumbar segment:there was no relation(P>0.05);SS,LL,PI were positively correlated with CSA of lumbar paravertebral muscle and CSA of vertebral body(P<0.01);④The correlation between PI-LL and CSA of lumbar paravertebral muscles at each lumbar segment:there was no correlation in group A(P>0.05);In group B,there was no correlation between PI-LL and L_(3~4),L_(4~5)lumbar paravertebral muscle CSA/vertebral body CSA(P>0.05),there was a weak negative correlation with L_(5)~S_(1)lumbar paravertebral muscle CSA/vertebral body CSA(P<0.05);In group C,PI-LL was moderately correlated with L_(3~4)and L_(4~5)lumbar paravertebral muscle CSA/vertebral body CSA(P<0.05),and weakly correlated with L_(5)~S_(1)lumbar paravertebral muscle CSA/vertebral body CSA(P<0.05).Conclusions With th
