摘　　要：目的探讨斯氏针辅助后外侧入路全髋关节置换术(THA)治疗股骨颈骨折的疗效。方法将60例行THA治疗的股骨颈骨折患者按照随机数字表法分为观察组(采用斯氏针辅助后外侧入路THA治疗,30例)与对照组(采用常规后外侧入路THA治疗,30例)。记录两组切口长度、术中出血量、手术时间、术后引流量。采用改良Harris评分评价髋关节功能。结果患者均获得6个月随访。术中出血量、手术时间、术后引流量:观察组明显少(短)于对照组(P<0.05)。切口长度两组比较差异无统计学意义(P>0.05)。改良Harris评分:术后3个月观察组明显高于对照组(P<0.05);术后6个月两组比较差异无统计学意义(P>0.05)。结论采用斯氏针辅助后外侧入路THA治疗股骨颈骨折,具有术中出血量少、手术时间短、术后引流量少的优点,更利于患者早期髋关节功能恢复。Objective To investigate the effect of posterolateral approach total hip arthroplasty(THA)assisted by Steinmann pin in the treatment of femoral neck fracture.Methods The 60 cases of femoral neck fractures treated with THA were randomly divided into the observation group(30 cases were treated with posterolateral approach THA assisted by Steinmann pin)and the control group(30 cases were treated with conventional posterolateral approach THA).The incision length,intraoperative bleeding volume,operation time and postoperative drainage volume of two groups were recorded.The function of hip joint was evaluated with modified Harris score.Results All patients were followed up for 6 months.The observation group was significantly less(shorter)than the control group in the intraoperative bleeding volume,operation time and postoperative drainage volume(P<0.05).There was no statistical difference in incision length between the two groups(P>0.05).Modified Harris score:at 3 months after operation,the observation group was significantly higher than the control group(P<0.05);at 6 months after operation,there was no statistical difference between two groups(P>0.05).Conclusions For treatment of femoral neck fracture,THA via the posterolateral approach assisted by Steinmann pin has the advantages of less intraoperative bleeding,shorter operation time and less postoperative drainage volume,which is more conducive to the early recovery of hip joint function.
