克氏针钛缆张力带治疗尺骨鹰嘴骨折的疗效

摘　　要：目的探讨克氏针钛缆张力带治疗MayoⅡ型尺骨鹰嘴骨折的临床疗效。方法采用克氏针钛缆张力带治疗23例MayoⅡ型尺骨鹰嘴骨折患者。记录骨折愈合情况、关节活动度和术后并发症情况,采用肘关节功能Mayo评分评价疗效。结果患者均获得随访,时间3~16个月。切口均愈合,无感染、骨化性肌炎、创伤性关节炎、骨折端分离、克氏针及钛缆断裂等并发症发生。末次随访时骨折均愈合。术后3个月,肘关节屈曲125°~150°(133.5°±6.8°),前臂旋前65°~85°(76.0°±1.6°)、旋后70°~80°(79.0°±1.9°);肘关节功能Mayo评分为70~100分,其中优16例,良6例,中1例,优良率22/23。结论采用克氏针钛缆张力带治疗MayoⅡ型尺骨鹰嘴骨折固定效果较佳,功能恢复良好,可取得较满意效果。Objective To investigate the clinical effect of Kirschner wire titanium tension band in the treatment of olecranon fracture.Methods The 23 patients with Mayo type-Ⅱulnar olecranon fractures were treated with Kirschner wires titanium cable tension band.Fracture union,range of motion and postoperative complications were recorded,and the results were evaluated using the Mayo score.Results All patients were followed up for 3~16 months.All the incisions healed without infection,myositis ossificans,traumatic arthritis,fracture end separation,Kirschner wire and titanium cable breakage.All fractures healed at the last follow-up.Three months after surgery,the elbow joint flexion and extension of the affected side were 125°~150°(133.5°±6.8°),the forearm pronation was 65°~85°(76.0°±1.6°)and supination was 70°~80°(79.0°±1.9°).Mayo elbow joint scores ranged from 70~100 points,among them 16 cases were excellent,6 cases were good,and 1 case medium,the excellent and good rate was 22/23.Conclusions Kirschner wire titanium tension band internal fixation in the treatment of olecranon fracture can achieve satisfactory results,better fixation effect,with good functional recovery.
