双切口复位内固定治疗三踝骨折

摘　　要：目的采用双切口复位内固定治疗三踝骨折的疗效。方法采用踝后外侧及内侧双切口复位内固定治疗21例三踝骨折患者。记录骨折愈合情况、并发症发生情况及踝关节功能恢复情况。结果患者均获得随访,时间6~18个月。切口均愈合,未出现感染、下肢深静脉血栓形成等并发症。骨折均骨性愈合,时间3~7个月。术后3个月,2例因不配合术后锻炼,踝关节背伸10°~15°、跖屈20°~30°;其余19例踝关节背伸20°~25°、跖屈40°~45°,踝关节功能基本恢复正常。末次随访时患者均可完全负重行走。结论采用踝后外侧及内侧双切口复位内固定治疗三踝骨折,手术操作简单,创伤小,并发症少,临床疗效较好。Objective To investigate the effect of double-incision reduction and internal fixation in the treatment of trimalleolar fracture.Methods The 21 patients with trimalleolar fractures were treated by posterolateral and medial double-incision reduction and internal fixation.The fracture healing,complications and functional recovery of ankle joint were recorded.Results All patients were followed up for 6~18 months.All incisions healed without infection or thrombosis of the lower limbs.All the fractures healed in 3~7 months.Three months after the operation,2 patients did not cooperate with postoperative exercise,who had ankle dorsiflexion of 10°~15°and plantar flexion of 20°~30°,the other 19 cases had ankle dorsiflexion of 20°~25°and plantar flexion of 40°~45°,the function of ankle joints returned to normal.At the last follow-up,all patients could walk with full weight-bearing.Conclusions The treatment of trimalleolar fracture by posterior lateral and medial double-incision reduction and internal fixation is simple,less trauma,few complications and better clinical effect.
