镇肝熄风汤加减联合针刺四花穴对缺血性脑卒中阴虚风动型神经功能缺损及预后的影响分析

摘　　要：目的探讨镇肝熄风汤加减联合针刺四花穴对缺血性脑卒中阴虚风动型患者神经功能缺损及预后效果分析。方法选择医院2017年4月—2021年4月收治的急性缺血性脑卒中阴虚风动型患者进行研究,采用随机数字表分为研究组和对照组,每组45例,对照组采取常规西医治疗,研究组在对照组的基础上,给予镇肝熄风汤加减联合针刺四花穴治疗,两组均治疗4周,比较临床疗效、同型半胱氨酸(homocysteine,Hcy)、超敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)及肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平、神经功能缺损(National Institute of Health stroke scale,NIHSS)评分以及改良Rankin量表(mRS)评分。结果研究组与对照组总有效率分别为91.11%(41/45)和73.33%(33/45),差异有统计学意义(P<0.05),研究组治疗后Hcy、hs-CRP及TNF-α水平均低于对照组,差异有统计学意义(P<0.05),NIHSS评分和mRS评分均低于对照组,差异具有统计学意义(P<0.05),两组不良反应情况差异无统计学意义(P>0.05)。结论镇肝熄风汤加减联合针刺四花穴治疗可明显改善缺血性脑卒中阴虚风动型患者神经功能,提高预后恢复水平,安全性高,适合推广应用。Objective To explore the effect of Zhengan Xifeng Decoction(镇肝熄风汤)combined with acupuncture at Sihua acupoints on the neurological deficit and prognosis of patients with ischemic stroke.Methods The patients with acute ischemic stroke with stirring wind due to Yin deficiency who were treated in the hospital from April 2017 to April 2021 were selected for re⁃search,and were divided into a study group and a control group by random number table,45 cases in each group.The control group received conventional western medicine.On the basis of the control group,the study group was given Zhengan Xifeng De⁃coction combined with acupuncture at Sihua acupoints.Both groups were treated for 4 weeks,and the clinical efficacy,homocys⁃teine(Hcy),hypersensitivity C reaction protein(hs-CRP),tumor necrosis factor-α(TNF-α)levels,neurological deficit(NIHSS)scores and modified Rankin Scale(mRS)scores were compared.Results The total effective rates of the study group and the control group were 91.11%(41/45)and 73.33%(33/45),respectively,and the difference was statistically significant(P<0.05).The levels of Hcy,hs-CRP and TNF-αin the study group were significantly lower than those in the control group after treatment(P<0.05).NIHSS score and mRS score in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Zhengan Xifeng Decoction combined with acupuncture at Sihua acupoints can significantly improve the neurological function of patients with ischemic stroke with stirring wind due to Yin deficiency,and improve the prognosis recover⁃y.It has high safety and is suitable for popularization and application.
