安神合剂与黛力新合用对脑卒中后抑郁-焦虑共病神经功能缺损、负性心理及不良反应的影响

摘　　要：目的探讨脑卒中后抑郁-焦虑共病(post-stroke anxiety combined with depression,PSCAD)患者合用安神合剂与黛力新治疗对其神经功能缺损、负性心理及不良反应的影响。方法选取2019年8月—2021年5月医院收治PSCAD患者146例,随机数字表法分对照组及观察组,各73例。对照组予以黛力新治疗,观察组在对照组基础之上联用安神合剂;比较治疗前与治疗4周后两组中医证候相关积分,酶联免疫分析仪检测血清5-羟色胺(5-hydroxytryptamine,5-HT)、盐酸多巴胺(Dopamine hydrochloride,DA)以及去甲肾上腺素(Norepinephrine,NE)水平,美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评估神经功能缺损现象,选取特质焦虑分量表(Trait anxiety invento⁃ry,T-AI)、状态焦虑分量表(S-AI)和汉密尔顿抑郁量表与(Hamilton Depression Scale,HAMD)进行负性心理评估;比较两组治疗后不良反应的发生率。结果治疗后,观察组中医症候相关积分皆明显优于对照组(P<0.05);治疗后,观察组血清5-HT、DA及NE水平显著高于对照组,观察组NIHSS评分明显低于对照组(P<0.05);治疗后,观察组T-AI、S-AI和HAMD评分皆明显低于对照组(P<0.05)。观察组不良反应发生率低于对照组,但差异不存在统计学意义(P>0.05)。结论安神合剂与黛力新联用改善PSCAD患者神经功能缺损状况,有效调节负性心理,不增加不良反应发生率。Objective To investigate the effects of Anshen Mixture(安神合剂)and Deanxit on neurological deficit,negative psychology and adverse reactions in patients with post-stroke depression anxiety comorbidity(PSCAD).Methods A total of 146 patients with PSCAD treated in the hospital from August 2019 to May 2021 were randomly divided into control group and observa⁃tion group,with 73 cases in each group.The control group was treated with Deanxit,and the observation group was treated with Anshen Mixture on the basis of the control group.The TCM symptom related scores of the two groups before and 4 weeks after treatment were compared.The levels of serum serotonin(5-HT),dopamine hydrochloride(DA)and norepinephrine(NE)were detected by enzyme-linked immunosorbent assay.The neurological deficit was evaluated by the National Institutes of Health Stroke Scale(NIHSS),and the trait anxiety subscale(T-AI),State Anxiety Scale(S-AI)and Hamilton Depression Scale(HAMD)were used for negative psychological evaluation.The incidence of adverse reactions after treatment was compared be⁃tween the two groups.Results After treatment,the TCM syndrome related scores in the observation group were significantly better than those in the control group(P<0.05).After treatment,the levels of serum 5-HT,DA and NE in the observation group were significantly higher than those in the control group,and the NIHSS score in the observation group was significantly lower than that in the control group(P<0.05).After treatment,the scores of T-AI,S-AI and HAMD in the observation group were signifi⁃cantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion Anshen Mixture com⁃bined with Deanxit can improve the neurological deficit of PSCAD patients,effectively regulate negative psychology and do not in⁃crease the incidence of adverse reactions.
