2012—2021年深圳市宝安区早产儿发生率及变化趋势分析

摘　　要：目的分析深圳市宝安区早产儿的发生情况及变化趋势。方法利用深圳市妇幼保健管理信息系统,计算2012—2021年于宝安区出生的活产儿早产数和早产率,采用趋势χ2检验法对早产率随时间的变化趋势进行评估,同时计算不同社会人口学特征下早产率差异。结果2012—2021年期间宝安区监测活产儿483590例,其中早产儿29116例(6.02%),早产率从2012年的5.78%上升到2021年的6.98%,年平均增幅为2.12%,差异有统计学意义(趋势χ^(2)=104·192,P<0.01)。分组分析结果显示,早产率中产妇为流动人口(6.27%)高于户籍人口(5.96%)、年龄≥35岁(8.72%)高于<35岁(5.68%)、学历为高中及以下(6.17%)高于大专及以上(5.73%)、初检孕周≥13周(6.45%)高于<13周(5.68%)、母亲产检次数<5次(9.29%)高于≥5次(4.83%)。结论宝安区早产发生率呈上升趋势,加强高龄、流动孕产妇等高风险人群高危预警和全程管理,是宝安区控制早产儿发生,保障母婴安全工作的重点。Objective To analyze the occurrence of premature infants and time trends in Baoan district,Shenzhen,China.Methods Livebirths data from 2012‒2021 in Baoan district were obtained from Shenzhen Maternal and Child Health Management System to calculate the numbers and rates of premature infants,usingχ^(2)test to evaluate the change tendency of the preterm birth rate over time,while calculating the preterm birth rate difference under different social demographic characteristics.Results The overall rate of premature infants in Baoan district during 2012‒2021 was 6.02%(29116/483590),with significant increasing tendency,rising from 5.78%in 2012 to 6.98%in 2021,with the average annual rate of 2.12%.Subgroup analysis revealed that the rates of premature infants were higher among the following mothers:Migrants(6.27%),aged≥35 years(8.72%),lower educational level(6.17%),took the first prenatal examination after 13 weeks of gestation(6.45%)and antenatal visit less than 5 times(9.29%).Conclusion The preterm birth rate in Baoan district is on the rise.Strengthen the high-risk groups(such as advanced age and migrant population)for early risk warning and management all the way,is the key actions for controlling the occurrence of premature infants and guarantee the maternal and infant safety in Baoan district.
