苓桂术甘汤辅助治疗对慢性收缩性心力衰竭患者的疗效、心功能、心肌损伤、运动功能的影响

摘　　要：目的:探讨苓桂术甘汤辅助常规西药治疗慢性收缩性心力衰竭(Chronic systolic heart failure,CSHF)的疗效及对患者心功能、心肌损伤、运动功能的影响。方法:选择我院2020年3月至2021年8月收治的98例CSHF患者,根据治疗方案不同,分为常规治疗组(常规西药治疗)、中药辅助组(苓桂术甘汤辅助常规西药治疗),各49例。治疗1 m后,比较两组疗效、心功能相关指标[左心室舒张末内径(Left ventricular end diastolic diameter,LVEDD)、左心室的射血分数(Left ventricular ejection fraction,LVEF)、心输出量、每搏量]、心肌损伤指标、运动功能(6 min行走距离)、生活质量(明尼苏达心力衰竭生活质量问卷调查(Minnesota living heart failure questionnaire,MLHFQ)评分)、不良反应。结果:治疗后,中药辅助组总有效率高于常规治疗组(P<0.05);中药辅助组LVEDD、心肌损伤指标水平及MLHFQ评分低于常规治疗组(P<0.05),LVEF、心输出量、每搏量、6 min行走距离均高于常规治疗组(P<0.05);治疗期间,两组不良反应发生率无统计学差异。结论:苓桂术甘汤辅助常规西药治疗可明显提高CSHF患者的疗效、改善患者心功能、心肌损伤指标,提高运动能力和生活质量,且安全性较高。Objective:To investigate the therapeutic effect of Linggui Shugan decoction assisted conventional western medicine in the treatment of chronic systolic heart failure(CSHF)and its influence on cardiac function,myocardial injury and motor function.Methods:98 patients with CSHF admitted to our hospital from March 2020 to August 2021 were selected and divided into routine treatment group(conventional western medicine)and traditional Chinese medicine(TCM)auxiliary group(Linggui Shugan decoction assisted conventional western medicine)according to different treatments,with 49 patients in each group.After 1 month of treatment,the efficacy,cardiac function related indicators[left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),cardiac output,stroke volume],myocardial injury indicators,motor function(6 min walking distance),Minnesota living heart failure questionnaire(MLHFQ)scores,and adverse reactions were compared between routine treatment group and TCM auxiliary group.Results:After treatment,the total effective rate of the TCM auxiliary group was higher than that of the conventional treatment group(P<0.05);LVEDD,myocardial injury indicators,MLHFQ score in the TCM auxiliary group were lower than those in the routine treatment group(P<0.05),LVEF,cardiac output,stroke volume,and 6-minute walking distance were higher than those in the routine treatment group(P<0.05);during the treatment,the incidence of adverse reactions had no significant difference between the two groups(P>0.05).Conclusion:Linggui Shugan decoction assisted with conventional western medicine can significantly improve the curative effect of chronic CSHF,improve their cardiac function,myocardial injury indicators,and improve their exercise ability and quality of life,with high safety.
