微创切口异体韧带重建治疗慢性踝关节外侧不稳定的临床研究

摘　　要：目的:探讨微创切口进行异体韧带重建治疗慢性踝关节外侧不稳定的疗效。方法:分析2020年12月至2022年12月期间采用微创切口异体韧带重建手术治疗慢性踝关节外侧不稳定的26例患者的临床资料,测量对比术前术后距骨前移距离及距骨倾斜角度,以美国骨科足踝协会(American Orthopaedic Foot and Ankle Society)踝-后足功能评分评价踝关节功能及疼痛情况。组间比较采用配对样本t检验。结果:26例患者均获得随访,终末随访时前抽屉试验距骨前移[(3.65±0.56)mm],低于术前[(12.27±1.51)mm,t=28.320,P<0.01];距骨倾斜角度[(3.41±0.79)°],低于术前[(7.14±1.05)°,t=15.590,P<0.01];AOFAS踝-后足功能评分由术前的(57.15±4.25)分提高到术后终末随访时的(91.42±3.28)分(t=34.346,P<0.01),差异均有统计学意义。所有患者无严重并发症出现,对疗效满意。结论:微创切口异体韧带重建治疗慢性踝关节外侧不稳定的手术方式创伤小,术后恢复快,无严重并发症,踝关节功能明显改善。Objective To explore the efficacy of minimally invasive incision for allogeneic ligament reconstruction in the treatment of chronic lateral ankle instability.Methods The clinical data of 26 patients with chronic lateral instability of the ankle treated with minimally invasive allogeneic ligament reconstruction surgery from December 2020 to December 2022 were analyzed.The distance of talus anterior displacement and talus tilt angle before and after surgery were measured and compared.The ankle joint function and pain were evaluated using the AOFAS ankle hindfoot function score.The measurement data were expressed as mean±standard deviation(mean±SD)and compared using paired sample t-test.Results All 26 patients were followed up,and at the end of the follow-up,the anterior drawer test showed a forward displacement of the talus[(3.65±0.56)mm],which was lower than that before surgery[(12.27±1.51)mm,t=28.320,P<0.01].The inclination angle of the talus was(3.41±0.79)°,less than preoperation[(7.14±1.05)°,t=15.590,P<0.01].The AOFAS ankle posterior foot function score increased from(57.15±4.25)before surgery to(91.42±3.28)at the end of postoperative follow-up,with a statistically significant difference(t=34.346,P<0.01).All patients had no serious complications and were satisfied with the therapeutic effect.Conclusion Minimally invasive incision allogeneic ligament reconstruction for the treatment of chronic lateral instability of the ankle joint has minimal trauma,fast postoperative recovery,no serious complications,and significant improvement in ankle joint function.
