摘　　要：目的:经导管主动脉瓣置入术已经成为高危及不能外科手术的主动脉瓣狭窄患者的主要治疗方式,其在全世界范围内的临床应用迅猛发展,其术后并发症的观察和护理是协助患者顺利康复的重要一环,本研究主要探讨经导管主动脉瓣置入术治疗老年患者主动脉瓣膜病变术后并发症的护理方法与效果。方法:回顾性选取2017年3月至2020年10月,在安贞医院瓣膜外科中心接受经导管主动脉瓣置入术的主动脉瓣狭窄患者的临床资料,总结并分析接受经导管主动脉瓣置入术后患者的并发症及护理经验。结果:共完成经导管主动脉瓣置入术180例,14例患者出现轻度瓣周漏,15例出现Ⅲ°房室传导阻滞,9例置入永久起搏器,1例出现肺部感染,2例出现血管并发症,3例出现下肢静脉血栓。结论:针对接受经导管主动脉瓣置入术患者常见并发症进行密切观察,给予针对性的护理干预,降低并发症的发生,是保障患者顺利出院的关键。Objective:Transcatheter aortic valve placement has become the main treatment for patients with aortic stenosis at high risk of inoperable surgery,and its clinical application is rapidly developing worldwide.The observation and care of postoperative complications is an important part of assisting patients to recover successfully,and the study focused on the nursing methods and effects of transcatheter aortic valve placement for postoperative complications in elderly patients with aortic valve lesions.Methods:The medical data of patients with aortic stenosis who underwent transcatheter aortic valve placement in Beijing Anzhen Hospital from March 2017 to October 2020 were retrospectively selected to summarize and analyze the complications and nursing experience of patients who underwent transcatheter aortic valve placement.Results:A total of 180 transcatheter aortic valve replacements were completed in our department.14 patients had mild perivalvular leaks,15 hadⅢ°atrioventricular block,9 had permanent pacemakers placed,1 had pulmonary infection,2 had vascular complications,and 3 had lower extremity venous thrombosis.Conclusions:A close observation of common complications in patients undergoing transcatheter aortic valve placement and targeted nursing interventions to reduce the occurrence of complications are essential to ensure successful discharge.
