四妙勇安汤对糖尿病足部溃疡患者血清lncRNA MALAT1表达量及足背血流动力学的影响

摘　　要：目的:分析四妙勇安汤对糖尿病足部溃疡患者血清长链非编码核糖核酸(lncRNA)肺癌转移相关转录本1(MALAT1)表达量及足背血流动力学的影响。方法:选取150例糖尿病足部溃疡患者进行回顾性研究,根据治疗方法将其分为两组,每组75例,对照组给予常规西药治疗,观察组在对照组基础上给予四妙勇安汤治疗,比较两组临床疗效、症状体征评分、血清lncRNA MALAT1表达量、血清炎性因子、足背血流动力学指标、肉芽组织形成时间、创面愈合时间。结果:观察组临床总有效率高于对照组(P<0.05)。观察组治疗后色泽、渗出、疼痛、面积积分低于对照组(均P<0.05)。观察组治疗后血清lncRNA MALAT1表达量、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平低于对照组(均P<0.05)。观察组治疗后足背动脉血管内径高于对照组,足背动脉血流速度低于对照组(均P<0.05)。观察组肉芽组织形成时间、创面愈合时间短于对照组(均P<0.05)。结论:四妙勇安汤可有效缓解糖尿病足部溃疡患者坏疽、疼痛等症状,降低血清lncRNA MALAT1表达量,减轻炎症反应,改善足部血液循环,促进肉芽组织生长,缩短创面愈合时间。Objective:To analyze the effects of Simiao Yong’an decoction on serum long chain non-coding RNA(lncRNA)metastasis-associated lung adenocarcinoma transcript 1(MALAT1)expression and dorsal foot hemodynamics in patients with diabetic foot ulcer.Methods:150 patients with diabetic foot ulcers were selected for retrospective study,and they were divided into two groups according to different treatment methods,75 cases in each group.The control group was given conventional western medicine treatment,and the observation group was given Simiao Yong’an decoction on the basis of the control group.The clinical efficacy,symptom and sign scores,serum lncRNA MALAT1 expression,serum inflammatory factors,dorsal foot hemodynamic indexes,granulation tissue formation time and wound healing time were compared between the two groups.Results:The total clinical effective rate of observation group was higher than that of control group(P<0.05).The scores of color,exudation,pain and area in the observation group were lower than those in the control group after treatment(all P<0.05).After treatment,the serum lncRNA MALAT1 expression,tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)in observation group were lower than those in control group(all P<0.05).The internal diameter of dorsal foot artery in the observation group was higher than that in the control group,and the blood flow velocity of dorsal foot artery in the observation group was lower than that in the control group(all P<0.05).Granulation formation time and wound healing time of observation group were shorter than those of control group(all P<0.05).Conclusion:Simiao Yong’an decoction can effectively relieve gangrene,pain and other symptoms in diabetic foot ulcers,reduce serum lncRNA MALAT1 expression,reduce inflammatory response,improve foot blood circulation,promote the growth of granulation tissue,and shorten wound healing time.
