肠梗阻胎儿围产期不良妊娠结局的高危因素分析

摘　　要：目的 探讨影响肠梗阻胎儿围产期不良结局的高危因素。方法 选取2015-01—2021-12在郑州大学第三附属医院产科分娩的先天性肠梗阻胎儿102例,根据围产期是否出现并发症分为不良结局组和对照组。比较2组患儿临床特征及围产期资料,Logistic回归分析肠梗阻胎儿发生围产期不良结局的高危因素。结果 不良结局组产前胎儿缺氧的发生率及Ⅳ型肠闭锁的占比明显高于对照组,差异有统计学意义(P<0.01)。Logistic回归分析显示,肠闭锁Ⅳ型及产前胎儿缺氧是肠梗阻胎儿发生不良结局的高危因素(P<0.05)。结论 肠闭锁Ⅳ型及产前胎儿缺氧是肠梗阻胎儿围产期发生不良结局的高危因素。Objective To investigate the risk factors of perinatal complications in fetuses with congenital intestinal obstruction.Methods A total of 102 fetuses with congenital intestinal obstruction who were delivered in the Obstetrics Department of the Third Affiliated Hospital of Zhengzhou University from January 2015 to December 2021 were selected.According to whether the fetus had perinatal complications or not,they were divided into poor outcome groups.and no adverse outcome group.The clinical characteristics and perinatal data of the two groups were compared,and the high risk factors of perinatal complications in fetus with intestinal obstruction were analyzed by Logistic regression.Results The incidence of prenatal fetal hypoxia and the proportion of type IV intestinal atresia in the adverse outcome group were significantly higher than those of the no adverse outcome group,and the difference was statistically significant(P<0.01).Logistic regression analysis showed that intestinal atresia type IV and prenatal fetal hypoxia were risk factors for poor outcomes in fetus with intestinal obstruction(P<0.05).Conclusion Intestinal atresia type IV and antenatal fetal hypoxia are high risk factors for adverse perinatal outcomes in fetuses with intestinal obstruction.
