自体血液回输技术应用于剖宫产术中的可靠性研究

摘　　要：目的探讨自体血液回输技术应用于剖宫产术中应用效果及可靠性。方法选取2019年7月-2021年12月贵州医科大学附属白云医院收治的80例产后出血患者,以随机数字表法分为对照组和观察组,各40例。对照组采用异体血输注,观察组采用自体血液回输技术,比较两组围术期输血及康复情况、血常规指标、血流动力学指标、输血不良反应。结果观察组术中出血量、异体输血量、术后入住ICU时间、住院时间均低于对照组(P<0.05);观察组术后24 h血红蛋白(Hb)、红细胞压积(Hct)、纤维蛋白原(Fib)水平高于对照组(P<0.05);观察组术后24 h血流动力学指标包括收缩压(SBP)、舒张压(DBP)、经皮血氧饱和度(Tcso2)高于对照组,心率(HR)低于对照组(P<0.05);观察组输血不良反应发生率为7.50%,低于对照组的22.50%(P<0.05)。结论自体血液回输技术在剖宫产术中有良好应用效果,能够减少患者术中出血量和异体输血量,改善患者血常规指标和血流动力学指标,缩短患者住院时间,促进患者术后康复,降低患者的输血反应,是一种安全可靠的输血方式。Objective To investigate the effectiveness and reliability of autologous blood transfusion technology in cesarean section.Methods A total of 80 patients with postpartum hemorrhage admitted to Baiyun Hospital Affiliated to Guizhou Medical University from July 2019 to December 2021 were selected and divided into control group and observation group by random number table method,with 40 patients in each group.The control group was treated with allogeneic blood transfusion,and the observation group was treated with autologous blood transfusion.Perioperative blood transfusion and rehabilitation,blood routine indexes,hemodynamic indexes and adverse reactions of blood transfusion were compared between the two groups.Results The intraoperative blood loss,allogeneic blood transfusion volume,postoperative ICU stay and hospitalization time in the observation group were lower than those in the control group(P<0.05).The levels of hemoglobin(Hb),hematocrit(Hct)and fibrinogen(Fib)in the observation group were higher than those in the control group at 24 hours after operation(P<0.05).The hemodynamic indexes including systolic blood pressure(SBP),diastolic blood pressure(DBP)and transcutaneous oxygen saturation(Tcso2)in the observation group were higher than those in the control group,and the heart rate(HR)was lower than that in the control group(P<0.05).The incidence of adverse reactions of blood transfusion in the observation group was 7.50%,which was lower than 22.50%in the control group(P<0.05).Conclusion Autologous blood transfusion technology has a good application effect in cesarean section,which can reduce the amount of intraoperative blood loss and allogeneic blood transfusion,improve the blood routine indexes and hemodynamic indexes,shorten the hospitalization time,promote the postoperative rehabilitation,and reduce the transfusion reaction.It is a safe and reliable blood transfusion method.
