摘　　要：目的分析开封市新报告经性传播HIV/AIDS病例及时抗病毒治疗(antiretroviral therapy,ART)情况及影响因素,为提高ART及时性,遏制艾滋病经性传播提供科学依据。方法收集2016-2022年开封市HIV/AIDS病例人口学特征、样本来源、报告地、报告单位、报告时疾病状态、首次CD4+T淋巴细胞计数(CD4)等信息,采用logistic回归分析ART及时性影响因素。结果开封市2016-2022年997例病例中449例ART及时,ART及时率45.0%,2016-2022年每年ART及时率分别为:23.7%、28.6%、47.8%、58.2%、55.1%、61.2%、42.5%,ART及时率呈逐年增高趋势(χ^(2)趋势=34.732,P<0.001)。多因素logistic回归分析结果显示,确证年、报告地、现居住地、报告时疾病状态为ART及时性的影响因素。与2016年比较,2018年(OR=3.245,95%CI:1.955~5.386)、2019年(OR=5.138,95%CI:3.080~8.572)、2020年(OR=4.561,95%CI:2.684~7.752)、2021年(OR=5.882,95%CI:3.439~10.061)和2022年(OR=2.500,95%CI:1.477~4.233)ART及时率高;与本市报告比较,本省其他地市报告(OR=0.629,95%CI:0.441~0.899)、外省报告(OR=0.370,95%CI:0.216~0.635)ART及时率低;与现居住地为市区比较,现居住地为县(OR=0.336,95%CI:0.238~0.475)ART及时率低;与HIV病例相比,AIDS病例(OR=1.522,95%CI:1.097~2.112)ART及时率高。结论近年来开封市经性传播HIV/AIDS病例ART及时性逐年提高,但仍待加强,提高ART及时性需要高度关注流动、现居住地为县、HIV感染者,开展专项宣传和针对性措施,提高病例ART及时性,遏制开封市艾滋病经性传播。Objective To analyze the timeliness of antiretroviral therapy(ART)for newly reported transmissible HIV/AIDS cases in Kaifeng and the influencing factors,so as to provide the scientific evidence for improving the timeliness of ART and stopping transmissible HIV transmission.Methods Demographic characteristics,sample source,reporting site,reporting institution,dis ease status at the time of reporting,and first-time CD4+T lymphocyte count(CD4)of HIV/AIDS cases in Kaifeng city from 2016-2022 were collected,and logistic regression was used to analyze ART timeliness and influencing factors.Results Of 997 cases in Kaifeng from 2016-2022,449 took ART timely,with an ART timeliness rate of 45.0%,and the annual ART timeliness rates from 2016-2022 were:23.7%,28.6%,47.8%,58.2%,55.1%,61.2%and 42.5%,respectively,with an increasing trend in ART timeliness(mm=34.732,P<0.001).Multivariate regression analysis showed that year of confirmation,place of reporting,current residence,and disease status at the time of reporting were influential factors for ART timeliness.ART timeliness rates for cases reported in 2018(OR=3.245,95%CI:1.955-5.386),2019(OR=5.138,95%CI:3.080-8.572),2020(OR=4.561,95%CI:2.684-7.752),2021(0R=5.882,95%CI:3.439-10.061)and 2022(OR=2.500,95%CI:1.477-4.233)were higher than that in 2016.ART timeliness rates for cases reported form other cities in the province(OR=0.629,95%CI:0.441-0.899)and out of province(OR=0.370,95%Cl:0.216-0.635)were higher than that from the local city.ART timeliness rate was lower in the cases currently living in counties(OR=0.336,95%CI:0.238-0.475)than that in the urban areas;the ART timeliness rate was higher in AIDS cases(OR=1.522,95%CI:1.097-2.112)than that in HIV cases.Conclusions Timeliness of ART for sexually transmitted HIV/AIDS cases in Kaifeng has improved in recent years,but it still needs to be strengthened.Improving the timeliness of ART requires to pay high attention to floating cases,cases currently residing in counties and HIV-infected cases,special publicity and targeted measures
