造口袋联合Ⅳ3000透明敷贴对减少医用黏胶相关性皮肤损伤的效果

摘　　要：目的:探究造口袋联合Ⅳ3000透明敷贴使用,在减少医用黏胶相关性皮肤损伤(medical adhesive related skin injuries,MARSI)的效果。方法:选取我院神经外科ICU收治的158例大便失禁(fecal incontinence,FI)患者,随机分为2组,每组79例。对照组采用造口袋,并进行相关护理,试验组在此基础上联合Ⅳ3000透明敷贴。1个月后,统计患者肛周护理时间及MARSI发生情况。评估患者肛周皮肤损害程度及患者的舒适度。结果:试验组肛周护理时间为(30.59±5.24)min,MARSI发生率为10.13%,均低于对照组的肛周护理时间(42.26±5.15)min和MARSI发生率26.58%(P<0.05);2组FI患者压疮预后积分表(PUSH)均升高(P<0.05),其中试验组PUSH评分低于于对照组(P<0.05);2组FI患者舒适状况量表评分均升高(P<0.05),其中试验组评分高于对照组(P<0.05)。结论:造口袋联合Ⅳ3000透明敷贴能够缩短FI患者的肛周护理时间,降低MARSI的发生,减少对肛周皮肤的损伤,同时提高患者的舒适度。Objective:To investigate the effect of ostomy bag combined with IV3000 transparent applicator in reducing medical adhesive-related skin injury(MARSI).Methods:From January 2020 to May 2022,158 patients with fecal incontinence(FI)admitted to the neurosurgery ICU of Jiangsu Provincial Hospital were selected and divided into two groups by random number table method,with 79 cases in each group.All patients were given ostomy bags,the control group was given routine care,and the observation group was givenⅣ3000 transparent applicator plus routine care.The perianal nursing time and the occurrence of MARSI were recorded after one month of intervention,and the degree of perianal skin damage and the patient’s comfort were evaluated.Results:The perianal nursing time was 30.59±5.24 min and the incidence of MARSI was 10.13%in the observation group,both of which were lower than the 42.26±5.15 min and 26.58%in the control group(P<0.05).After nursing,the pressure ulcer prognostic score scale(PUSH)of FI patients in both groups increased(P<0.05),and the PUSH score in the observation group was lower than that in the control group(P<0.05).The scores of comfort scale of FI patients in both groups were increased(P<0.05),and the scores in the observation group were higher than those in the control group(P<0.05).Conclusion:Ostomy bag combined withⅣ3000 transparent dressing can shorten the perianal nursing time of FI patients,decrease the occurrence of MARSI,reduce the damage to the perianal skin,and improve the comfort of patients.
