摘 要:目的探讨电针联合刮痧疗法对颈型颈椎病家兔颈肌炎症因子IL-1β、IL-6、TNF-α表达及NF-κB/IκB/IKKβ信号通路的影响。方法将40只新西兰家兔随机分为正常组、模型组、电针组、刮痧组、联合组,每组各8只。采用寒湿刺激加长期低头位的方法建立颈型颈椎病家兔模型。造模8周后,电针组选取双侧颈夹脊穴进行电针治疗;刮痧组选取督脉及两侧足太阳膀胱经刮痧治疗;联合组给予电针联合刮痧治疗。于造模前、造模后进行颈椎X线拍摄并评分。治疗15 d后留取家兔颈部两侧后伸肌,采用苏木精-伊红(HE)染色观察肌肉组织形态的变化。实时荧光定量PCR法(qRT-PCR)检测颈肌IL-1β、IL-6、TNF-α、NF-κB p65、IκB、IKKβmRNA表达水平。Western blot法检测颈肌IL-1β、IL-6、TNF-α、NF-κB p65、IκB、IKKβ蛋白表达水平。结果与正常组比较,造模各组家兔X线评分均增高(均P<0.05)。与模型组比较,各治疗组家兔颈肌的炎性损伤均有所改善,其中联合组改善最明显。各治疗组IL-1β、IL-6、TNF-α、NF-κB p65、IKKβmRNA和蛋白表达水平低于模型组(均P<0.05),且联合组低于电针组、刮痧组(均P<0.05)。各治疗组IκB mRNA和蛋白表达高于模型组(均P<0.05),联合组高于电针组、刮痧组(均P<0.05)。结论电针联合刮痧疗法可改善颈部肌肉的病理损伤,作用优于单独电针或刮痧治疗。其作用机制可能是通过下调NF-κB/IκB/IKKβ信号通路相关因子的表达,进而降低IL-1β、IL-6、TNF-α等炎症因子的表达。Objective To investigate the effects of electroacupuncture combined with scraping therapy on the signaling pathways of inflammatory factors IL-1β,IL-6,TNF-αand NF-κB/IκB/IKKβin cervical muscle cells of rabbits with cervical spondylosis.Methods Forty rabbits were divided into 4groups randomly,including normal group,model group,electroacupuncture group,scraping group and combined group,with 8rabbits in each group.These rabbits were treated with cold-dampness stimulation and long-term bowed head position method to construct cervical spondylosis model.After 8weeks,bilateral cervical Jiaji acupoint intervention was performed in the electroacupuncture group.Du pulse and bladder meridian were selected for intervention in the scraping group.Combination of scraping and electroacupuncture therapy were performed in the combined group.The cervical X-ray of each rabbit was taken and scored before and after molding.After 15days of treatment,the posterior extensor muscles were collected from rabbits’bilateral neck muscles.Hematoxylin-eosin staining(HE)was used to observe the morphological changes of muscle.The expression levels of IL-1β,IL-6,TNF-α,NF-κB p65,IκB and IKKβmRNA in neck muscle were determined by real-time quantitative PCR.The expression levels of IL-1β,IL-6,TNF-α,NF-κB p65,IκB and IKKβproteins in neckpuncture combined with scraping therapy can improve the pathological injury of neck muscle,and the effect is better than that of electroacupuncture or scraping alone.Its mechanism of action may be through downregulating the expression of factors related to NF-κB/IκB/IKKβsignaling pathway,and then reducing the release of IL-1β,IL-6,TNF-αand other inflammatory factors.
