清热类中药饮片用药安全与警戒分析

摘　　要：目的:分析清热类中药饮片的用药安全,并进行警戒思考,为中药饮片的合理用药提供参考。方法:通过检索文献国内外数据库2015-2022年关于15味使用较多的清热类中药饮片出现不良反应的文献,采用Excel 2017和统计学软件回顾性分析清热类中药饮片出现不良反应的关联性、临床表现及原因。结果:临床中出现过的不良反应共30例,其中出现不良反应的中药饮片共8味,分别为决明子、黄连、土茯苓、穿心莲、生石膏、黄芩、夏枯草、山豆根。临床中出现过的不良反应30例的关联性评价中,予以肯定的例数为26例,占比为86.67%。30例不良反应中山豆根出现不良反应的例数最多,占比26.67%。中药饮片出现不良反应的原因中药物因素以及剂量不合理占比最大,分别为40.00%、26.67%。结论:部分清热类中药饮片中的不良反应较为突出,需要针对出现不良反应的原因进行警戒分析,以最大限度保证中药饮片的用药安全性。Objective:To analyze the medication safety of heat clearing Chinese herbal decoction pieces,and conduct alert thinking to provide reference for the rational use of Chinese herbal decoction pieces.Methods:By searching the literature from domestic and foreign databases from 2015 to 2022 on the adverse reactions of 15 commonly used heat-clearing traditional Chinese medicines.Excel 2017 and statistical software were used to retrospectively analyze the correlation,clinical manifestations,and causes of adverse reactions of heat-clearing traditional Chinese medicines.Results:A total of 30 cases of adverse reactions had been reported in clinical practice,8 pieces of traditional Chinese medicine with adverse reactions,including cassia seed,coptis chinensis,tuckahoe,andrographis paniculata,gypsum,scutellaria baicalensis,prunella vulgaris,and yam root.In the correlation evaluation of 30 clinical adverse reactions,26 cases were confirmed,accounting for 86.67%.Among the 30 cases of adverse reactions,Zhongshan Dougen had the highest number of adverse reactions,accounting for 26.67%.Drug factors and unreasonable dosage accounted for the largest proportion of adverse reactions in traditional Chinese medicine slices,40.00%and 26.67%,respectively.Conclusion:Adverse reactions in some heat-clearing Chinese herbal medicines are prominent,and alert analysis should be conducted on the causes of adverse reactions to maximize the safety of Chinese herbal medicines.
