摘 要:目的:分析万古霉素骨水泥在开放性跟骨骨折患者术后感染治疗中的应用效果。方法:选择我院2019年3月-2022年3月收治的62例跟骨骨折患者为研究对象,将其随机分成对照组和研究组,每组31例。对照组进行常规治疗,研究组在对照组基础上应用万古霉素骨水泥进行填充治疗,比较两组临床疗效、炎症指标[肿瘤坏死因子(TNF-α)、白介素-6(IL-6)、中性粒细胞、白细胞、C反应蛋白(CRP)]以及临床指标(换药次数、创面愈合恢复时间、感染控制时间、住院时间)。结果:研究组治疗有效率为96.77%,高于对照组的77.42%(P<0.05);两组治疗后TNF-α、IL-6、中性粒细胞、白细胞、CRP水平低于治疗前,且研究组低于对照组(P<0.05);研究组创面愈合恢复时间、感染控制时间、住院时间短于对照组,换药次数少于对照组(P<0.05)。结论:应用万古霉素骨水泥能有效控制开放性跟骨骨折患者术后感染,且具有较好的炎症抑制作用,值得临床应用。Objective:To analyze the effect of vancomycin bone cement on postoperative infection in patients with open calcaneal fracture.Methods:A total of 62 patients with calcaneal fractures admitted to our hospital from March 2019 to March 2022 were randomly divided into control group and study group,with 31 cases in each group.The control group was treated with routine treatment,and the study group was treated with vancomycin cement on the basis of the control group.The clinical efficacy,inflammatory indicators[tumor necrosis factor(TNF-α),interleukin-6(IL-6),neutrophils,white blood cells,C-reactive protein(CRP)]and clinical indicators(number of dressing changes,wound healing recovery time,infection control time,hospitalization time)were compared between the two groups.Results:The effective rate of treatment in the study group was 96.77%,which was higher than 77.42%in the control group(P<0.05).After treatment,the levels of TNF-α,IL-6,neutrophils,white blood cells and CRP in the two groups were lower than those before treatment,and those in the study group were lower than the control group(P<0.05).The wound healing recovery time,infection control time and hospitalization time of the study group were shorter than those of the control group,and the number of dressing changes was less than that of the control group(P<0.05).Conclusion:Vancomycin cement can effectively control postoperative infection in patients with open calcaneal fractures,and has a good inhibitory effect on inflammation,which is worthy of clinical application.
