摘　　要：目的:探讨体位干预联合穴位推拿对极低出生体重(VLBW)/超低出生体重(ELBW)早产儿呼吸支持的应用效果。方法:选取2020年3月至2021年3月徐州医科大学附属滕州市中心人民医院VLBW/ELBW早产儿78例,利用数字表法将其分为对照组与观察组,各39例。对照组给予常规护理方法,观察组在此基础上实施体位干预联合穴位推拿护理。观察并对比两组早产儿临床结局、生长发育及胃肠功能的变化。结果:干预后,观察组早产儿的无创通气时间等临床结局显著优于对照组,早产儿各项生长发育指标明显优于对照组,早产儿的胃动素及胃泌素等功能指标有效改善于对照组,差异均有统计学意义(均P<0.05)。结论:对VLBW/ELBW早产儿实施体位干预联合穴位推拿护理可有效改善其胃肠功能及各项生长发育指标,进而减少住院及用氧时间,对促进良好的临床结局伴有一定积极作用。Objective To explore the application effect of posture intervention combined with acupoint massage on respiratory support for very low birth weight(VLBW)/extremely low birth weight(ELBW)premature infants.Methods A total of 78 VLBW/ELBW premature infants in Tengzhou Central People's Hospital Affiliated to Xuzhou Medical University from March 2020 to March 2021 were selected and divided into control group and observation group with 39 cases in each group by digital table method.The control group was given routine nursing methods,while the observation group was given posture intervention combined with acupoint massage nursing on this basis.The clinical outcome,the growth and development and gastrointestinal function of the two groups were observed and compared.Results After intervention,clinical outcomes such as non-invasive ventilation time of preterm infants in the observation group were significantly better than those in the control group,the growth and development indicators of preterm infants in the observation group were significantly better than those in the control group,and functional indicators such as motilin and gastrin of preterm infants were effectively improved compared with the control group,with statistical significance(P<0.05).Conclusions The implementation of posture intervention combined with acupoint massage care for VLBW/ELBW premature infants can effectively improve their gastrointestinal function and various growth and development indicators,and then reduce hospitalization and oxygen use time,which is associated with a positive effect on promoting good clinical outcomes.
